双眼白内障患者两眼手术疼痛差异的比较

Comparing anesthetic methods for cataract surgery

目的探讨双眼白内障患者两眼分次手术时的疼痛差异及对策。方法对40例双眼白内障患者,间隔1~3周分别行两眼白内障超声乳化吸除联合人工晶状体植入术,采用视觉模拟评分法(VAS)观测两次手术过程中的麻醉镇痛效果并记录分析。结果两眼手术时间及眼别比差异无统计学意义(P>0.05);VAS评分显示患者第1眼手术疼痛程度均低于第2眼,麻醉过程及手术过程评分比较,两眼差异有统计学意义(P<0.05)。两眼手术总体满意度对比差异有统计学意义(P<0.05);两组的无明显不良反应发生率差异无统计学意义(P>0.05)。结论双眼白内障患者分次手术的第2眼手术时的疼痛程度明显高于第1眼,不存在眼别差异,有必要实施干预,提高患者手术满意度。

Objective To explore the difference in pain and management following cataract surgery in patients treated for both eyes.Methods 40 cataract patients received phacoemulsification and intraocular lens implantation for both eyes.One eye was operated first,and the other eye was operated in 1 to 3 weeks.Effects of analgesic were recorded using a visual analogue scale(VAS)during the surgeries and were analyzed.Results There was no difference in surgery time(P>0.05).The VAS scores for the first surgery were lower than that for the second surgery(P<0.05),including anesthesia scores and surgical procedure scores.There was significant difference in overall satisfaction(P<0.05).No significant difference in the incidence of adverse events(P>0.05).Conclusions For patients with cataracts in both eyes,pain in the second eye surgery is significantly higher.It is necessary to implement preoperative interventions to improve patient satisfaction.
