乳腺癌保乳加前哨淋巴结活检术患者的护理

Nursing care of breast cancer patients undergoing breast conserving plus sentinel lymph node biopsy

目的探讨乳腺癌保乳加前哨淋巴结活检术患者的护理。方法将2015年9月~2017年9月在我院外科行乳腺癌保乳加前哨淋巴结活检术的76例患者随机分为两组,对照组采用常规护理方法,观察组采用综合护理方法,比较两组患者的术后恢复时间、并发症发生率、治疗依从性及护理满意率、术后生活质量。结果观察组术后腋窝引流时间、术后疼痛持续时间、住院时间与对照组相比明显减少,Neer肩关节功能评分明显高于对照组,差异有统计学意义(P<0.05);观察组护理后上肢麻木、肿胀、感觉减退、切口感染等并发症发生率明显低于对照组,差异有统计学意义(P<0.05);观察组治疗依从性及护理满意率明显高于对照组,差异有统计学意义(P<0.05);观察组护理2周后生理职能、生理功能、疼痛、活力、社会功能、精神健康、情感职能、总体健康等SF-36评分明显高于对照组,差异有统计学意义(P<0.05)。结论乳腺癌保乳加前哨淋巴结活检术患者采用围术期综合护理效果显著,能有效促进术后康复,减少并发症的发生,加快肩关节功能的恢复,提升术后生活质量,具有积极的临床意义。

Objective To explore the nursing care of breast cancer patients undergoing breast conservation plus sentinel lymph node biopsy.Methods Seventy-six cases who underwent breast conserving surgery and sentinel lymph node biopsy in our hospital from September 2015 to September 2017 were randomly divided into two groups,the control group adopted conventional nursing methods while the observation group adopted comprehensive nursing methods.The recovery time,complication rate,treatment compliance,nursing satisfaction rate and postoperative quality of life of the two groups were compared.Results The postoperative axillary drainage time,postoperative pain duration and hospitalization time in the observation group were significantly reduced compared with those in the control group,and neer shoulder joint function score was significantly higher than that in the control group,the difference was statistically significant(P<0.05).The incidence of upper limb numbness,swelling,hypoesthesia,incision infection and other complications in the observation group was significantly lower than that in the control group after nursing(P<0.05).The treatment compliance and nursing satisfaction rate in the observation group were significantly higher than those in the control group(P<0.05).The SF-36 scores of physiological function,physiological function,pain,vitality,social function,mental health,emotional function and overall health in the observation group were significantly higher than those in the control group after 2 weeks of nursing(P<0.05).Conclusion The comprehensive perioperative nursing care for breast cancer patients undergoing breast-conserving surgery plus sentinel lymph node biopsy is effective.It can effectively promote postoperative recovery,reduce complications,accelerate the recovery of shoulder joint function,and improve the quality of life after surgery.It has positive clinical significance.
