腹腔镜与开腹手术治疗急性阑尾炎的临床疗效及安全性比较

Comparison of clinical efficacy and safety between laparoscopy and laparotomy in the treatment of acute appendicitis

目的比较分析腹腔镜与开腹手术治疗急性阑尾炎的临床疗效及安全性。方法选取2015年11月~2017年4月期间我院接诊的164例急性阑尾炎患者,应用随机抽样法分为研究组和对照组。其中82例作为研究组,采用腹腔镜手术进行治疗;另外82例作为对照组,采用开腹手术进行治疗。比较分析两组患者治疗后临床疗效及安全性。结果研究组患者治疗后的手术时间、出血量、胃肠功能恢复时间、肛门排气时间、疼痛持续时间及住院时间显低于对照组,且两组患者护理后手术指标和恢复情比较差异有统计学意义(P<0.05)。两组患者治疗后均发生不同程度的并发症,且研究组患者治疗后的并发症发生率明显低于对照组,且两组患者治疗后并发症发生情况差异有统计学意义(P<0.05)。结论我院采取的腹腔镜手术治疗急性阑尾炎具有明显的优势,其治疗效果及安全性明显高于开腹手术,是治疗急性阑尾炎非常有效的方法,值得在临床中广泛应用。

Objective To compare the clinical efficacy and safety of laparoscopy and laparotomy in the treatment of acute appendicitis.Methods One hundred and sixty-four cases of acute appendicitis admitted to our hospital from November 2015 to April 2017 were randomly divided into the study group and the control group.Among them,82 cases were treated by laparoscopic surgery as the study group.The other 82 cases were treated with laparotomy as the control group.The clinical efficacy and safety of the two groups after treatment were compared and analyzed.Results The time of operation,the amount of bleeding,the recovery time of gastrointestinal function,the time of anus exhaust,the duration of pain and the time of hospitalization in the study group were significantly lower than those in the control group,and there was statistically significant difference between the two groups in the operation index and recovery situation after nursing(P<0.05).Two groups of patients had different degrees of complications after treatment.The incidence of complications after treatment in the study group was significantly lower than that in the control group.There was a significant difference in the incidence of complications between the two groups after treatment(P<0.05).Conclusion The laparoscopic operation in our hospital has obvious advantages in the treatment of acute appendicitis.Its therapeutic effect and safety are obviously higher than that of laparotomy.It is a very effective method for the treatment of acute appendicitis,and it is worth widely used in clinical practice.
