高危型HPV感染对宫颈病变组织中P53蛋白表达的影响

摘要 目的分析高危型HPV感染对P53表达的影响,探究高危型HPV感染在宫颈病变发生发展过程中的作用机制。方法选取自2015年1月~2017年9月间妇科活检、LEEP刀切除、宫颈锥切及子宫全切标本75例纳入研究,并根据病情进行分组,分为CINI组(n=13)、CINⅡ组(n=26)、CINⅢ组(n=39),另选取同时间段来院进行健康体检的30例自愿者作为对照组。四组均采用免疫组化法检测P53的表达,采用PCR(聚合酶链反应)检测宫颈分泌物中高危型HPV DNA的载量。P53蛋白在正常宫颈组织中及CIN的表达、高危HPV感染在宫颈病变组织中的表达以及高危HPV检测与P53蛋白表达之间的关系。结果与对照组相比,P53蛋白在CINI、CINⅡ及CINⅢ三组宫颈病变患者中的表达水平有显著提高,差异有统计学意义(P<0.05);与对照组相比,高危HPV感染程度在CINI、CINⅡ及CINⅢ三组宫颈病变患者中的表达水平有显著提高,差异有统计学意义(P<0.05);随着高危型HPV感染程度的增加与P53蛋白表达的增多,宫颈病变程度也随之增加,差异无统计学意义(P>0.05)。结论 P53的异常表达在宫颈病变早期,发挥着重要的作用。P53的表达与高危型HPV感染程度存在相关性,高危型HPV与P53可能存在协同作用。 Objective To analyze the effect of high-risk HPV infection on the expression of P53 and explore the mechanism of high-risk HPV infection in the development of cervical lesions.Methods From January 2015 to September 2017,75 cases of gynecologic biopsy,LEEP knife resection,cervical conization and total hysterectomy were studied and divided into groups according to the disease analysis.and were divided into two groups according to the disease analysis,divided into CINI group(n=13),CIN group(n=26),CIN group(n=39),30 cases were selected at the same time to the hospital for health examination as the control group.The expression of P53 was detected by immunohistochemistry in four groups.PCR(polymerase chain reaction)was used to detect the load of high-risk HPV DNA in cervical secretions.Expression of P53 protein in normal cervical tissues and CIN,the expression of high risk HPV infection in cervical lesions and the relationship between high risk HPV detection and P53 protein expression.Results Compared with the control group,the expression level of P53 protein in patients with cervical lesions of CINⅠ,CINⅡand CINⅢwas significantly increased(P<0.05).Compared with the control group,the high-risk HPV infection was The expression levels of CINⅠ,CINⅡand CINⅢin patients with cervical lesions were significantly increased(P<0.05).With the increase in the level of high-risk HPV infection and the increase of P53 protein expression,the extent of cervical lesions was also increased.With the increase,the difference was not statistically significant(P>0.05).Conclusion The abnormal expression of P53 plays an important role in the early stage of cervical lesions.The expression of P53 is correlated with the degree of high risk HPV infection,and high risk HPV may have synergistic effect with P53.
