髌软骨T2 mapping与膝关节骨性关节炎临床症状间的关系及影响因素

Relationship between T2 mapping of Patellar Cartilage and Clinical Symptoms of Osteoarthritis of Knee Joint and its Influencing Factors

摘要 目的探讨膝骨关节炎患者髌软骨T2 mapping与临床症状、关节软骨缺损及骨髓病变间的关系。方法回顾性选取2017年1月~7月本院32例符合1986年美国风湿学会症状性膝关节骨性关节炎患者,行膝关节MRI检查,包括T2WI、3D FSPGR和T2 mapping,评估髌软骨缺损及骨髓病变分级,在T2 mapping图像上勾画ROI,测量髌软骨T2值,以T2值中位数为区分标准,作各项指标的两组间比较,Logistic回归分析骨髓病变、软骨缺损与T2值间的相关性,行方差分析观察软骨缺损及骨髓水肿与T2值间的关系。结果软骨缺损及骨髓病变在两组间有统计学差异,P值均<0.05。Logistic回归分析发现软骨缺损及骨髓病变与T2值是否高于中位数相关,P值分别为0.003及0.032;不同等级关节软骨缺损及骨髓病变间T2值不同,且呈现增高趋势,P值均<0.05。结论不同等级的软骨缺损及骨损伤具有不同的T2值,且随着等级的增高,T2值亦呈现增高趋势。 Objective To investigate the relationship between T2 mapping of patellar cartilage and clinical symptoms,articular cartilage defects and bone marrow lesions in patients with knee osteoarthritis.Methods From January to July 2017,32 patients with symptomatic osteoarthritis of knee joint,who were in accordance with the 1986 American Rheumatology Society,were examined with MRI,including T2WI,3DFSPGR and T2 mapping.The patellar cartilage defects and bone marrow lesions were evaluated.To evaluate the grade of patellar cartilage defect and bone marrow lesion,draw ROI on T2 mapping image,measure T2 value of patellar cartilage,and use median T2 value as the distinguishing criterion.The relationship between bone marrow lesion,cartilage defect and T2 value was compared between the two groups.The relationship between cartilage defect,bone marrow edema and T2 value was observed by ANOVA.Results There was significant difference in cartilage defect and bone marrow lesion between the two groups,P values were all<0.05.Logistic regression analysis showed that the correlation between cartilage defect and bone marrow lesion and T2 P values were 0.003 and 0.032 respectively.Different grades of articular cartilage defects and bone marrow lesions showed different T2 values and showed an increasing trend.P values were all<0.05,respectively.Conclusion Different grades of cartilage defects and bone injuries have different T2 values,and with the increase of grade,T2 values also show an increasing trend.
