中药骨蚀灵胶囊对早期兔激素性股骨头坏死的干预作用

Intervention research on Chinese medicine Gushiling capsule in treating steroid-associated necrosis of the femoral head in rabbits

目的探讨骨蚀灵胶囊对早期兔激素性股骨头坏死(SANFH)脂质代谢变化特点和其防治作用的机制。方法 48只日本大耳兔随机分为对照组、模型组、骨蚀灵胶囊组、洛伐他汀组。除对照组外,使用大剂量激素构建股骨头坏死模型,并按照实验计划进行药物干预。在第2、4周测定各组血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白(HDL)和低密度脂蛋白(LDL)水平,观察并分析各组核磁共振图像和组织病理学改变。结果第2周时,与对照组相比,其余三组的TC、TG、LDL均明显升高(P<0.01),而HDL明显降低(P<0.01);第4周时,骨蚀灵胶囊组、洛伐他汀组兔血清中的TC、TG、LDL水平较模型组明显降低(P<0.05,P<0.01),但仍高于对照组(P<0.01)。病理切片及核磁检测显示模型组出现明显的股骨头坏死表现,而骨蚀灵胶囊组和洛伐他汀组病理改变较模型组轻。结论骨蚀灵胶囊可早期调节机体血清脂质水平,降低血脂,从而起到预防和缓解股骨头坏死进展的作用。

Objective To investigate the early changes in lipid metabolism characteristics of Gushiling cap-sule on steroid-associated necrosis of the femoral head(SAFNH)and its mechanisms for treating steroid-associated necrosis of the femoral head.Methods Forty-eight rabbits were randomly assigned to four groups:the blank group,model group,Gushiling capsule group and lovastatin group.In addition to the blank group,SANFH models were prepared by injection of high dose prednisolone gluteus and then intervention according to the experimental plan.Determining the serum lipid level of TC,TG,HDL and LDL in each group rabbits after treated 2 and 4 weeks,obser-vation and analysis of each group of magnetic resonance images and histopathological changes.Results On the second week,the levels of TC,TG and LDL in the other three groups were significantly higher than those in the control group(P<0.01),while those in the control group were significantly lower(P<0.01);On the fourth weekend,the levels of TC,TG and LDL in the serum of Gushiling capsule group and lovastatin group rabbits were significantly lower than those in the model group(P<0.05,P<0.01),but still higher than those in control group(P<0.01).Pathological and nuclear magnetic detection showed that the necrosis of the femoral head appeared in the model group,but the pathological changes in Gushiling capsule group and lovastatin group were lighter than those in the model group.Conclusion Gushiling capsule can regulate the serum lipid level and reduce the blood lipid of the rabbit model of SANFH in the early stage,which can prevent and alleviate the progress of the femoral head necrosis.
