初产妇分娩时会阴侧切与自然裂伤对产妇盆底功能的影响被引量：2

摘要目的探究初产妇分娩时会阴侧切与自然裂伤对产妇盆底功能的影响.方法选取我院2016年1月至2016年8月收诊的88例经阴道分娩的初产妇,按照分娩状况分为会阴侧切组(A组)和会阴自然裂伤组(B组)各44例,分别于产后42天、6个月时返院,检测盆底功能,包括盆底肌力、压力性尿失禁发生率、盆腔器官脱垂率(阴道前后壁脱垂、子宫脱垂).结果产后42d后,A组盆腔器官脱垂率33.34%高于B组14.58%,尿失禁发生率31.25%高于B组12.50%,6个月后A组盆腔器官脱垂率10.42%高于B组4.17%,A组尿失禁发生率10.42%(5/48)高于B组2.08%(2/48),差异有统计学意义(P<0.05);产后42d、6个月后,A组盆底肌力正常率20.83%低于B组54.17%,差异有统计学意义(P<0.05).结论适当进行会阴侧切手术可避免初产妇在分娩时产生严重会阴撕裂,但会阴侧切手术对产妇盆底功能影响较大,应谨慎选择.
