改良的BRADEN与APACHE Ⅱ评分预警压疮高危患者压疮发生的临床研究

Clinical study of improved BRADEN and APACHE Ⅱscore in predicting pressure ulcers in patient with high risk of pressure ulcer

目的探讨改良的压疮评分量表(Braden评分)与急性生理学及慢性健康状况评分(APACHE Ⅱ评分)预警压疮高危患者压疮发生的关系。方法将该院2017年10月至2018年7月收治的100例Braden评分小于或等于12分患者作为研究对象,随机筛选出Braden评分在6~<10分患者50例作为压疮高危组,Braden评分在10~<12分患者50例作为压疮危险组,分析比较发生与未发生压疮患者APACHE Ⅱ评分、亚项目得分。结果平均收缩压是影响患者压疮发生的独立危险因素(P<0.01),患者心率,睁眼反应、语言反应、运动反应评分与压疮的发生无关(P>0.05)。结论改良的Braden评分及APACHE Ⅱ评分预警压疮高危患者压疮的发生率较高,能够提高医护人员对压疮的防护能力,从而可以有针对性地进行压疮的个体化预防,节约医疗资源,值得临床推广运用。

Objective To investigate the relationship between improved BRADEN and APACHEⅡscore in patients with high risk of pressure sore.Methods The patients with Braden score less than or equal to 12 points of the hospital from October 2017 to June 2018 were selected as the research subjects,and 50 cases of Braden score in 6<10 patients were randomly selected as high risk group for pressure sore,and 50 cases of Braden in 10<12 patients were used as the risk group of pressure sore,and the scores of APACHEⅡand sub item scores of patients with non pressure sores were compared.Results Mean systolic blood pressure is an independent risk factor affecting pressure ulcers(P<0.01).Patients'eye opening reaction,language reaction and motor response scores were non related to pressure sore(P>0.05).Conclusion The improved Braden score and APACHE II score have high incidence of pressure sore in high risk patients with high risk of pressure sore,which can improve the protection ability of the medical staff to pressure sore,thus it can be targeted to carry out individual prevention of pressure sore,save medical resources and be worthy of clinical application.
