严重粉碎髋臼骨折手术治疗的效果及对患者髋关节功能的影响

The Effect of Surgical Treatment of Severe Comminuted Acetabular Fracture and Its Effect on the Hip Function of the Patients

目的:分析严重粉碎髋臼骨折手术治疗的效果及对患者髋关节功能的影响。方法:选取本院在2016年2月-2017年9月收治的100例严重粉碎髋臼骨折患者,按数字法随机分为对照组和研究组,每组各50例,其中对照组患者采用K-L入路,研究组患者采用联合入路,治疗后观察对比两组患者疼痛评分、手术治疗效果、患者髋关节活动度,同时对影响疗效的指标进行单因素进行回归分析。结果:单因素分析结果显示患者性别、年龄、手术入路与手术疗效无明显关系(P>0.05),而复位质量、骨折类型、手术时机以及异位骨化与手术疗效有关(P<0.05);研究组患者髋关节优良率与对照组比较差异无统计学意义(P>0.05);研究组患者髋关节活动度与对照组比较差异无统计学意义(P>0.05);对照组患者疼痛评分与研究组比较差异无统计学意义(P>0.05)。结论:严重粉碎髋臼骨折手术治疗的临床效果更佳,能够积极促进患者髋关节功能的康复,显著减少患者疼痛,值得在临床上进一步采用和推广。

Objective:To analyze the effect of surgical treatment for severe comminuted acetabular fracture and its effect on the hip function of the patients.Method:100 cases with severe comminuted acetabular fracture in our hospital from February 2016 to September 2017 were randomly divided into control group and study group,50 cases in each group,the control group was treated by K-L approach,the study group was treated with combined approach.After treatment,the pain scores,effects of surgical treatment,and patients with hip the joint activity of two groups were observated and compared.At the same time,the effects of the index of single factor regression was analysed.Result:The univariate analysis showed no significant relationship of the sex,age,surgical approach and surgical effect(P>0.05);the quality of reduction,fracture type,timing of surgery and surgical treatment of heterotopic ossification had relationship with the surgical effect(P<0.05);the excellent rate of hip joint in two groups with no significant difference(P>0.05);the hip joint activity of two groups had no significant difference(P>0.05);the control group showed no obvious difference in pain scores with the study group(P>0.05).Conclusion:Surgical treatment of severe comminuted acetabular fractures has a better clinical effect.It can actively promote the rehabilitation of hip function and significantly reduce the pain of patients,which is worthy of further promotion and application in clinic.
