社区健康管理对慢性病防治的效果观察

Effect observation of community health management on the prevention and treatment of chronic diseases

目的:探讨社区健康管理对慢性病防治的效果。方法:收治慢性病患者126例,给予社区健康管理,比较患者干预前后的效果。结果:与干预前比较,干预后患者的慢性病行为明显改善,差异有统计学意义(P<0.05)。干预后患者的血糖达标、血压达标、血脂达标、慢性病知识知晓及非药物治疗知晓情况显著提升,差异有统计学意义(P<0.05)。结论:社区健康管理能够有效改善慢性病患者的不良生活习惯,降低由生活方式导致的慢性病的发病率。

Objective:To explore the effect of community health management on the prevention and treatment of chronic diseases.Methods:126 cases of patients with chronic diseases were given with community health management,and the effect of the patients before and after intervention was compare.Results:Compared with the chronic disease behavior before intervention,the patients'chronic disease behavior was significantly improved,and the difference was statistically significant(P<0.05).The blood glucose standard,blood pressure standard,blood lipid standard,knowledge of chronic disease knowledge and knowledge of non-drug treatment after intervention were significantly improved,and the difference was statistically significant(P<0.05).Conclusion:Community health management could improve the bad habits of patients with chronic disease effectively and reduce the incidence of chronic diseases caused by lifestyle.
