重度子痫前期患者凝血功能的变化及意义

Changes and significance of coagulation function in the patients with severe preeclampsia

目的探讨凝血功能指标在重度子痫前期患者及正常妊娠妇女中的表达与意义。方法选择在我院就诊的重度子痫前期患者30例作为实验组,同期正常妊娠妇女40例为对照组,对两组进行凝血酶原时间(PT)、凝血酶原时间国际标准化比值(INR)、活化部分凝血酶时间(APTT)、凝血酶时间(TT)、D-二聚体(D-D)的检测,并用免疫比浊法进行血管性血友病因子(vWF)值检测,对比分析两组孕妇的妊娠结局。结果实验组PT、INR、APTT、及TT低于对照组,而D-D和vWF高于对照组,差异具有统计学意义(P<0.05);实验组胎儿生长受限、早产率明显高于对照组。结论重度子痫前期孕妇血液呈高凝状态可导致不良妊娠结局,通过凝血指标及vWF的检测可以作为初步判断病情严重程度的指标之一。

Objective To investigate the expression and significance of coagulation function parameters in the patients with severe preeclampsia and normal pregnant women.Methods 30 patients with severe preeclampsia who were treated in our hospital were selected as the experimental group.During the same period,40 normal pregnant women were selected in the control group.Prothrombin time(PT),international normalized ratio(INR)of prothrombin time,activated partial thrombin time(APTT),thrombin time(TT),and D-dimer(D-D)were measured in the two groups.The von Willebrand factor(vWF)value was detected by immunoturbidimetry and the pregnancy outcomes of pregnant women were analyzed and compared between the two groups.Results PT,INR,APTT,and TT were lower in the experimental group than in the control group.D-D and vWF were higher than the control group.The differences were statistically significant(P<0.05).The fetal growth restriction and premature birth rate in the experimental group were significantly higher than those in the control group.Conclusion The hypercoagulable state of blood in the pregnant women with severe preeclampsia may lead to adverse pregnancy outcomes.The detection of blood coagulation parameters and vWF can be used as one of the indicators for preliminarily judging the severity of disease.
