肥大细胞在HSV-2感染所致生殖器疱疹中的作用

Role of mast cells in genital herpes caused by HSV-2 infection

目的:检测生殖器疱疹患者外周血中肥大细胞数量、IL-33及IL-6的水平变化,探讨其在生殖器疱疹发病机制中的作用。方法:选择30例生殖器疱疹患者,采用口服伐昔洛韦片治疗,治疗前后流式细胞仪检测外周血中肥大细胞数量,ELISA检测IL-33、IL-6的水平,并与30例健康对照者进行比较。结果:生殖器疱疹患者经伐昔洛韦治疗后痊愈14例(46.7%),显效13例(43.3%),有效3例(10%),无效0例,总显效率为90%,无不良反应发生。治疗前生殖器疱疹患者外周血中肥大细胞数量较健康对照组明显增多(t=29.83,P<0.01),IL-33、IL-6水平升高(t值分别为170.88、25.03,P值均<0.01)。生殖器疱疹患者治疗后外周血中肥大细胞数量较治疗前明显减少(t=17.10,P<0.01),IL-33、IL-6水平较治疗前明显降低(t值分别为163.36、15.61,P值均<0.01)。结论:肥大细胞发挥其天然免疫应答作用,可能通过调节IL-33、IL-6参与HSV-2感染所致生殖器疱疹的发病机制。

Objective:To detect the changes in the number of mast cells,IL-33 and IL-6 levels in peripheral blood of patients with genital herpes,and to explore their role in the pathogenesis of genital herpes.Methods:Thirty patients with genital herpes were enrolled,treated by oral valaciclovir tablets.The number of mast cells and levels of IL-33 and IL-6 in peripheral blood of patients with genital herpes before and after treatment were determined using flow cytometry and ELISA,respectively,then compared with that of 30 normal controls.Results:Following the treatment with valacyclovir,total marked improvement rate was 90%(14 cured cases,13 markedly improved cases,3 improved cases),with no adverse events.The number of mast cells and levels of IL-33 and IL-6 in peripheral blood were significantly higher in patients with genital herpes before treatment than that in the normal controls(t=29.83,P<0.01).Treatments significantly lowered the number of mast cells(t=17.10,P<0.01)and levels of IL-33 and IL-6(t=163.36,15.61,respectively,P<0.01)in peripheral blood of patients with genital herpes.Conclusion:Mast cells exert a role in innate immune response,which may regulate IL-33 and IL-6 expression,leading to the development of genital herpes induced by HSV-2 infection.
