摘要
目的:观察CD1a、CD68抗原在湿疹患者皮损中的表达,探讨其临床意义。方法:采用免疫组织化学方法检测30例湿疹患者皮损表皮、真皮中CD1a、CD68抗原的表达,并与10例正常人皮肤进行比较。结果:正常皮肤中CD1a抗原主要表达于表皮棘层、基底膜朗格汉斯细胞膜上,而湿疹皮损中CD1a抗原主要散在分布于表皮棘层及真皮乳头中。CD68主要为单核-巨噬细胞膜染色,表达在表皮下部及真皮。湿疹皮损表皮及真皮中CD1a阳性、CD68阳性的细胞线性密度均明显高于正常皮肤(t值分别为2.86、4.43,P值均<0.01)。结论:CD1a、CD68抗原在湿疹患者皮损处表达较高,提示朗格汉斯抗原提呈细胞增多及单核-巨噬细胞浸润可能与湿疹发病有关。
Objective:To observe expression of CD1a and CD68 in eczematous skin.Methods:Immunohistochemical staining was used to detect expression of CD1a and CD68 antigens in both the epidermis and the dermis of lesional skin(30 patients)and skin of normal health controls(10 cases).Results:In normal controls,CD1a was mainly expressed on the membrane of Langerhans cells in the spinous layers and basal membrane,while in lesional skin of eczema,CD1a was mainly present in the spinous layers and the dermal papilla.CD68 staining was dominant on the membrane of mononuclear-macrophages in the lower epidermis and the dermis.The numbers of CD1a and CD68 positive cells in both the epidermis and the dermis of eczematous lesions were significantly higher than that in normal skin(t=2.86,4.43,respectively,P<0.01 for both).Conclusion:Expression levels of CD1a and CD68 antigens in eczematous lesions are elevated,suggesting that the increased number of antigen-presenting cells(Langerhans cells)and mononuclear-macrophage infiltration may be involved in the pathogenesis of eczema.
