卒中单元联合社区医疗服务模式对脑卒中患者临床效果观察

Clinical effect of apoplexy unit combined with community medical service model on stroke patients

目的探讨卒中单元联合社区医疗服务模式对脑卒中患者临床疗效。方法选取我院2015年7月~2017年7月收治的80例脑卒中患者为研究对象,按照随机化原则,将之分为卒中单元组、社区医疗服务模式组和卒中单元联合社区医疗服务模式组。卒中单元组经多个科室协助合作,诊治及后期监护、肢体、言语功能锻炼及心理康复治疗,同时涵盖中医范畴的诊治方法。社区医疗服务模式组,对有脑卒中高危人群进行高危因素进行预防及对于脑卒中的早诊早治。卒中单元联合社区医疗服务模式组,指在社区医疗服务模式上进行与卒中单元相同的诊治,即三级预防,尽早经卒中单元诊治后,早期进行康复治疗。分别从肢体活动功能,ADL能力及心理情况三方面比较三组的治疗效果。结果卒中单元联合社区医疗服务模式组分别与卒中单元组和社区医疗服务模式组比较,其在肢体运动功能评分与ADL能力评分上占有明显优势;在心理抑郁评定中明显好于后两组,经统计学分析,差异均有统计学意义(P<0.05)。三组患者的住院时间和住院费用比较差异均无统计学意义(P>0.05)。结论卒中单元联合社区医疗服务模式治疗脑卒中疗效好,对患者本人、社会及国家而言,皆符合当前国情,是具有中国特色的最佳医疗模式,可做为脑卒中的常规治疗模式,值得推广。

Objective To explore the clinical effect of apoplexy unit combined with community medical service model on stroke patients.Methods From July 2015 to July 2017,80 cases of cerebral apoplexy patients treated in our hospital were selected as the research object,and were divided into the apoplexy unit group,the community health service model group and apoplexy unit combined with community medical service model group according the principle of randomization.The apoplexy unit group has been assisted by multiple departments in cooperation,diagnosis and treatment,post hospital guardianship,physical and verbal functions,and psychological rehabilitation,and also includes the diagnosis and treatment methods of traditional Chinese Medicine.The community medical service model group was to prevent and treat early diagnosis and early treatment of cerebral apoplexy in high-risk groups with cerebral apoplexy.Apoplexy unit combined with community medical service model group,refers to the community health service model and the apoplexy unit on the same treatment,namely tertiary prevention,as soon as possible after diagnosis and treatment of apoplexy unit,early rehabilitation.The treatment effect of three groups were compared from the physical activity function,ADL ability and psychological condition.Results Compared with the apoplexy unit group and the community medical service mode group,the apoplexy unit combined with community medical service model group has a clear advantage in the physical function score and ADL ability score.It was better than the latter two groups in the evaluation of psychological depression(P<0.05).There was no significant difference between the three groups of patients'hospitalization time and hospitalization expenses(P>0.05).Conclusion The apoplexy unit combined with community medical service model is good for the treatment of stroke.It is in line with the current national conditions for patients,society and state.It is the best mode of medical treatment with Chinese characteristics.It can be used as a conventional treatment model for stroke.It is worth popularizing.
