纤维支气管镜支气管肺泡灌洗术治疗重症肺炎的临床效果分析

Clinical effect analysis of bronchoscopic bronchoalveolar lavage in the treatment of severe pneumonia

目的探讨纤维支气管镜支气管肺泡灌洗术治疗重症肺炎的临床效果分析。方法将2016年7月~2017年7月在我院ICU治疗的60例重症肺炎患者随机分为两组,对照组采用常规抗感染、辅助机械通气以及对症支持治疗,在此基础上观察组采用纤维支气管镜支气管肺泡灌洗术治疗,比较两组患者的临床疗效、各项症状改善时间、血气分析指标变化。结果观察组治疗有效率为93.33%,明显高于对照组的76.67%,差异有统计学意义(P<0.05);观察组治疗后症状消失时间、体温恢复正常时间、撤机时间、血象恢复正常时间、住院时间均明显较对照组缩短,差异有统计学意义(P<0.05);观察组治疗后PaO_2、PaCO_2、SpO_2、pH与对照组相比明显改善,差异有统计学意义(P<0.05)。结论纤维支气管镜支气管肺泡灌洗术治疗重症肺炎的临床效果显著,能迅速清除气道分泌物,快速改善症状,提高治愈率,缩短治疗时间,具有积极的临床意义。

Objective To investigate the clinical effect of bronchoalveolar lavage with fiberoptic bronchoscope in the treatment of severe pneumonia.Methods Sixty patients with severe pneumonia treated in ICU were randomly divided into two groups from July 2016 to July 2017,the control group was treated with routine anti-infection,auxiliary mechanical ventilation and symptomatic support therapy,on this basis,the observation group was treated by bronchoscopic bronchoalveolar lavage.The clinical efficacy,the symptoms improved time,blood gas analysis index changes compared of the two groups were compared.Results The effective rate of observation group was 93.33%,significantly higher than 76.67%in the control group,and the difference was statistically significant(P<0.05).After treatment,the symptoms disappear time,body temperature returned to normal time,withdraw machine time,blood picture back to normal time,shorten the length of hospital stay of observation group were significantly than those of control group,the difference was statistically significant(P<0.05).After treatment,the PaO2,PaCO2,SpO2 and pH were significantly improved compared with those of control group,with statistically significant difference(P<0.05).Conclusion Bronchoscopic bronchoalveolar lavage in the treatment of severe pneumonia has a significant clinical effect.It can quickly remove the airway secretions,improve the symptoms quickly,improve the cure rate and shorten the time of treatment.It has positive clinical significance.
