语前聋儿童人工耳蜗植入术后康复效果评估

Evaluation of Rehabilitation Effect after Cochlear Implantation in Children with Prelingual Deafness

目的探讨7岁以下语前聋儿童人工耳蜗植入后不同阶段的听觉、言语和交流康复效果及年龄、康复时间对效果的影响。方法选取50例7岁以下语前聋人工耳蜗植入儿童,根据人工耳蜗植入时的年龄分为≤3岁组(23例)及>3岁组(27例),分别于术后3、6、9、12个月进行听觉行为分级(categories of auditory performance,CAP)及言语可懂度分级(speech intelligibility rating,SIR)测试,比较两组对象的听觉能力、言语能力和交流能力。结果人工耳蜗植入术后1年内,随着植入时间的延长,两组对象的听觉能力、言语能力和交流能力均逐渐提高,且术后3、6、9、12个月,≤3岁组的听觉能力、交流能力均优于>3岁组(P<0.05),术后6、12个月,≤3岁组的言语能力优于>3岁组(P<0.05)。结论语前聋人工耳蜗植入儿童植入年龄越小、术后一年内康复训练时间越长,效果越好。

Objective To study the recovery effects of hearing,verbal and communication rehabilitative in 50 prelingually deafened cases under 7 years old for one year after cochlear implantation.Methods According to the implantation time,50 cases were divided into 2 groups:1 to 3 years old group(23 cases),and>3 to 7 years old group(27cases).The categories of auditory performance(CAP)and speech intelligibility rating(SIR)scores were used to evaluate the hearing,verbal and communication ability on 3,6,9,12 months after cochlear implataion.Results Within 1 year after cochlear implantation in 50 cases,the hearing ability,verbal ability and communication ability were gradually improved.Hearing ability and communication ability in the younger age group at the time of 3,6,9 and 12 months after CI surgery were higher than those of in the older group.The verbal ability in the younger age group on 6 and 12 months after CI surgery were higher than those of in the older group.Conclusion The younger when the children get cochlear implantation and the longer recovery time takes in one year after CI implantation,the better hearing effects will be.
