乳腺癌改良根治术联合前哨淋巴结活检治疗中青年乳腺癌的效果研究

Analysis of the effects of modified radical mastectomy combined with sentinel lymph node biopsy in the treatment of young and middle-aged patients with breast cancer

目的探究乳腺癌改良根治术联合前哨淋巴结活检治疗中青年乳腺癌的临床效果。方法选取2014年3月至2016年1月本院收治的210例中青年乳腺癌患者为研究对象,将其中采取乳腺癌改良根治术治疗并进行前哨淋巴结及腋窝淋巴结活检的110例患者纳入试验组,将仅采取乳腺癌改良根治术治疗的100例患者纳入对照组。比较两组患者肩关节外展角度变化情况、两侧上臂臂围差、肩关节功能评分及术后并发症发生情况。结果前哨淋巴结活检灵敏度为81.82%,特异度为100.00%;肿瘤部位、TNM分期及病理类型与前哨淋巴结活检结果均无相关性(P>0.05);治疗后,两组患者肩关节功能评分较治疗前均明显下降(P<0.05),两侧上臂臂围差较治疗前均显著增大(P<0.05);试验组患者术后并发症发生率明显低于对照组(P<0.05)。结论前哨淋巴结活检对临床诊断和预测中青年乳腺癌患者腋窝淋巴结转移具有较高的应用价值,部分乳腺癌患者可因此避免行腋窝淋巴结清扫,明显减少其术后并发症的发生。

Objective To analyze the effects of modified radical mastectomy combined with sentinel lymph node biopsy in the treatment of young and middle-aged patients with breast cancer.Method From March 2014 to January 2016,210 young and middle-aged patients with breast cancer in our hospital were selected as research objects,they were divided into trail group(110 cases)and control group(100 cases).Patients in trail group were treated with modified radical mastectomy,sentinel lymph node and axillary lymph node biopsy,and patients in control group were treated with modified radical mastectomy.The changes of angle of shoulder joint abduction,the differences of upper arm circumference of both sides,shoulder joint function scores,and postoperative complications were compared between the two groups.Result The sensitivity of sentinel lymph node biopsy was 81.82%and its specificity was 100.00%.There was no correlation between the tumor site,TNM stage and pathological type and the results of sentinel lymph node biopsy(P>0.05).After treatment,the shoulder function score was significantly lower than that of before treatment(P<0.05),and the difference of upper arm circumference of both sides was significantly higher than that of before treatment(P<0.05);the rates of postoperative complications in trail group was significantly lower than that of control group(P<0.05).Conclusion Sentinel lymph node biopsy has a high value in clinical diagnosis and prediction of axillary lymph node metastasis in young and middle-aged breast cancer patients,because of this,some breast cancer patients can avoid axillary lymph node dissection and significantly reduce postoperative complications.
