肝段切除患者围手术期序贯性肠内营养的疗效观察及护理

The Effect of Early Sequential Enteral Nutrition on Liver Resection Patients and Nursing

目的探讨肝段切除术后早期序贯性肠内营养的疗效及护理方法。方法选择2014年6月~2017年5月我院行肝段切除手术患者58例,随机分为对照组和干预组,各29例。干预组术后早期给予序贯性肠内营养(SEN)。对照组给予全胃肠外营养(TPN)。比较两组肠功能恢复时间、营养状况改善的差异。结果干预组术后首次肛门排气时间、排便时间短于对照组(P<0.05),术后8d前白蛋白恢复较对照组快,体重下降、上臂围减小幅度低于对照组(P<0.05)。结论 SEN在促进肠功能恢复、改善营养状况、促进肝脏蛋白质合成代谢方面优于TPN,是肝段切除术后患者安全、有效、合理的营养支持方式,有利于患者术后康复。

Objective To evaluate the effectiveness of the early sequential enteral nutrition on liver resection patients.Methods 58 cases of hepatectomy in our hospital from June 2014 to May 2017 were selected and randomly divided into two groups.29 cases in the intervention group were given sequential enteral nutrition(SEN)at early stage after operation,while 29 in the control group were given total parenteral nutrition(TPN).The recovery time of intestinal function and the difference of nutritional status were observed and compared between the two groups.Results Intestinal peristalsis and albumin restore faster in intervention group;weight loss and upper arm circumference decrease amplitude is lower than the control group(P<0.05).Conclusion Early sequential enteral nutrition is better than total parenteral nutrition in promote intestinal function recovery,improve nutritional status and promote the metabolism of liver protein synthesis.Early sequential enteral nutrition is safe,effective and reasonable way of nutritional support in patients with liver resection.
