胰岛素增敏剂在2型糖尿病强化治疗后长期缓解中的作用及对尿微量白蛋白的影响 被引量:3

摘要 目的:探讨胰岛素增敏剂在2型糖尿病强化治疗后长期缓解中的作用及对尿微量白蛋白的影响。方法:选择2015年4月到2016年4月我院接诊的80例2型糖尿病患者,通过随机数表法分为观察组(n=40)和对照组(n=40)。对照组采用二甲双弧治疗,观察组使用胰岛素增敏剂治疗。比较两组的血糖、胰岛素、C肽、HDL-C、LDL-C、Homa-IR指标和UAE水平。结果:观察组的FBG、PBG水平在治疗1、2月后均低于对照组(P<0.05);两组Hb A1c水平在治疗1、2月后无明显差异(P>0.05);两组FINS水平在治疗1、2月后均有所下降(P<0.05);治疗1、2月后,两组间FINS、PINS、FC-P及PC-P水平均无明显差异(P>0.05);观察组HDL-C指标在治疗后高于对照组[(1.51±0.55)mmo L/L vs(1.27±0.46)mmo L/L](P<0.05);观察组LDL-C指标在治疗后低于对照组[(2.92±0.71)mmo L/L vs(3.36±0.79)mmo L/L](P<0.05);观察组Homa-IR指标在治疗后低于对照组[(0.87±0.34)vs(1.31±0.34)](P<0.05);观察组UAE水平在治疗后低于对照组[(98.27±56.23)C/mg。(24h)-1vs(143.22±51.37)C/mg。(24h)-1](P<0.05)。结论:胰岛素增敏剂治疗2型糖尿病能有效降低患者血糖和FINS水平,使外周组织作用于胰岛素的敏感性得以改善,降低患者尿微量白蛋白水平,疗效显著,值得应用推广。 Objective:To study curative efficacy of insulin sensitizer in treatment of type 2 diabetes intensive treatment after long-term remission and its effects on urine microalbumin.Methods:80 patients of type 2 diabetes mellitus who received therapy from April 2015 to April 2016 were selected as research objects.According to random number table method,those patients were divided into the observation group(n=40)and the control group(n=40).The control group was treated with secondary double-arc treatment,while the observation group was treated with Insulin sensitizer treatment.Then blood sugar,insulin,C peptide,HDL-C,LDL-C,Homa-IR and UAE level were compared.Results:After treatment,the level of FBG and PBG in observation group was less than the control group after 1 month and 2 months(P<0.05).There was no significant difference in HbA1c levels between the two groups after 1 and 2 months of treatment(P>0.05).Two groups of FINS levels in the treatment of 1,2 months after the decline.There were no significant differences in FINS,PINS,FC-P and PC-P between the two groups after 1 and 2 months(P>0.05).HDL-C indicator in Observation group was higher than the control group[(1.51±0.55)mmol/L vs(1.27±0.46)mmol/L](P<0.05).LDL-C indicator in Observation group was less than the control group[(2.92±0.71)mmol/L vs(3.36±0.79)mmol/L](P<0.05).UAE level in Observation group was less than the control group[(98.27±56.23)C/mg.(24h)-1vs(143.22±51.37)C/mg.(24h)-1](P<0.05).Conclusion:Insulin sensitizer treatment of type 2 diabetes can effectively reduce the level of blood glucose and FINS,so that the sensitivity of peripheral tissue in the insulin to improve and reduce the level of urinary albumin in patients with significant efficacy,worthy of promotion.
