胰腺神经内分泌肿瘤的CT诊断及鉴别诊断 被引量:3

摘要 目的 探讨胰腺神经内分泌肿瘤(NETP)的CT图像特点及鉴别诊断方法。方法 收集本院2014年1月至2017年10月12例NETP患者的临床资料,回顾性分析其上腹部CT平扫联合增强图像特点。结果 12例均为单发,病理证实功能性NETP 5例,非功能性NETP 7例。术前CT正确诊断9例,符合率为75.0%。5例功能性NEPT均为实性,CT平扫等密度为主,增强动脉期中-重度强化,静脉期4例强化减弱,1例持续强化;7例非功能性NEPT,实性4例,囊实性2例,囊性1例,CT平扫等低密度为主,增强动脉期轻-重度强化,静脉期强化均减弱。2例伴钙化,3例伴邻近组织侵犯或远处转移。结论 胰腺神经内分泌肿瘤临床较罕见,功能性与非功能性NEPT的大小、形态、密度及与周围组织的比邻关系均具有一定特点。熟悉掌握这些特点,可为临床提供重要的影像学参考。 Objective To explore the CT characteristics and differential diagnosis methods of pancreatic neuroendocrine tumors(NETP).Methods The clinical data of 12 cases of NETP admitted into our hospital from January,2014 to October,2014 were collected.The CT scan and enhance image characteristics were retrospectively analyzed.Results All the 12 cases were single.5 cases of functional NETP and 7 cases of nonfunctional NETP were proven by pathology.9 cases(75.0%)were correctly diagnosed by CT.All the 5 cases of functional NEPT were solid,and the CT plain scan was equal density,witch expressed as mediate-severe reinforcement after arterial enhancement;4 cases were improved in venous period,and 1 case got continuous reinforcement.Among the 7 cases of nonfunctional NEPT,4 cases were solid,2 cases cystic-solid,and 1 cases cystic;the CT plain scan expressed as equal low density,witch expressed as mediate-severe reinforcement after arterial enhancement;all improved in venous period;2 cases got calcification,and 3 cases adjacent tissue invasion or distant metastasis.Conclusions Pancreatic neuroendocrine tumor is clinically rare.Functional and nonfunctional NEPT both have characteristics in size,shape,density,and distance relationship with the surrounding tissues.Mastery of these characteristics can provide important imaging for clinical reference.
