喉癌术后患者留置鼻饲管的影响因素分析与处理对策

Influencing factors and treatment countermeasures of nasal feeding tube in patients with laryngeal carcinoma after operation

目的 探讨喉癌术后患者留置鼻饲管的影响因素与处理对策。方法 选取本科2016年1月至2017年12月收治入院的喉癌术后留置鼻饲管患者60例作为本次研究对象,随机分为对照组与观察组,各30例。对照组留置胃管后,实施规范的健康教育和护理;观察组通过分析胃管使用的影响因素,结合临床及时采取了相应的对策。对两组患者的满意度、住院时间、非计划拔管、堵管、置管后并发症情况进行统计分析。结果 观察组患者干预后非计划拔管、堵管、置管后并发症发生例数为2例、2例、4例,明显低于对照组12例、8例、12例;住院时间(15.5±1.1)d明显短于对照组(21.6±1.3)d,患者满意度96.67﹪明显高于对照组73.33﹪,差异均有统计学意义(均P<0.05)。结论 加强患者宣教,发现问题及时进行护理干预,对术后患者鼻饲管的有效留置,保证机体足够的营养摄入,利于患者康复起到了重要的作用。

Objective To analyze the influencing factors and treatment countermeasures of nasogastric feeding tube in patients with laryngeal carcinoma after operation.Methods 60 patients with gastric tube were randomly divided into a control group and an observation group.After the control group had been indwelled stomach tubes,standardized health education and nursing care were implemented.The observation group were analyzed in time and the corresponding countermeasures were taken in combination with clinical factors by analyzing the influencing factors of gastric tube use.The statistical analysis was performed on the satisfaction of the groups,the length of stay,unplanned extubation,tube plugging,and complications after catheterization.Results After the intervention,2 cases occurred unplanned extubation,2 plugging,and 4 complications after tube insertion in the observation group;and 12,8,and 12 in the control group.The hospitalization time was significantly shorter and the satisfaction degree was significantly higher in the observation group than in the control group[(15.5±1.1)d vs.(21.6±1.3)d and 96.67%vs.73.33%],with statistical differences(P<0.05).Conclusions Strengthening patient education,discovering problems and conducting nursing interventions in time,effectively indwelling nasogastric feeding tubes for postoperative patients,ensuring sufficient nutrient intake of the body,and facilitating the recovery of the patient play important roles.
