前列地尔对糖尿病肾病患者纤维化发展的影响

Influence of alprostadil on fibrosis in patients with diabetic nephropathy

目的 探讨前列地尔对糖尿病肾病患者纤维化发展的影响。方法 选取本院2015年5月至2017年8月收治的98例糖尿病肾病患者进行研究,按随机数字表法将其分为研究组与对照组,各49例。对照组患者采用常规治疗,研究组在此基础上加用前列地尔治疗,治疗后两组患者治疗效果、肾功能、炎症反应、纤维化指标及不良反应发生情况比较。结果 研究组患者治疗总有效率91.83%,高于对照组73.46%,差异有统计学意义(P<0.05)。治疗前,两组患者纤维化指标、炎症因子水平、肾功能指标比较,差异无统计学意义(P>0.05);治疗后,研究组患者HA、MMP-9、PCIII、IVC、LN、IL-6及TNF-α水平均较对照组低,差异均有统计学意义(均P<0.05)。治疗后,BUN水平组间比较,差异无统计学意义(P>0.05);研究组Scr、β2-MG及UAER水平均低于对照组,差异有统计学意义(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论 前列地尔可有效缓解糖尿病肾病患者纤维化进程及炎症反应,改善肾功能,提高临床疗效,且安全性良好。

Objective To investigate the influence of alprostadil on fibrosis in patients with diabetic nephropathy.Methods 98 cases of patients with diabetic nephropathy in our hospital from May 2015 to August 2017 were chosen as experimental subjects,and were divided into control group and research group with the random number table,49 cases in each group.Both two groups were given routine treatment,and the research group was treated with alprostadil on the basis of routine treatment.After treatment,clinical effect,renal function,inflammatory reaction,fibrosis index,and adverse reactions were compared between the two groups.Results The total effective rate of the patients in the research group was 91.83%,which was higher than that in the the control group(73.46%),with statistically significant difference(P<0.05).There were no statistically significant differences in the indexes of fibrosis,the levels of inflammatory factors,renal function indexes between the two groups before treatment(P>0.05).After treatment,the levels of HA,MMP-9,PCIII,IVC,LN,IL-6,TNF-alpha,Scr,beta 2-MG,and UAER in the research group were lower than those in the control group,with statistically significant differences(P<0.05),while there was no statistically significant difference in the level of BUN(P>0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Alprostadil could effectively relief the fibrosis process and inflammatory reaction of patients with diabetic nephropathy,improve renal function and clinical effects,with good safety.
