医护卫联合门诊在高血压患者规范化管理中的应用效果

The application effect of Doctor-Nurse-Public Health Doctor joint outpatient service in standardized management of patients with hypertension

目的 评估社区医院开展医护卫联合门诊对高血压患者血压控制的效果。方法 选取2016年纳入本中心医护卫联合规范管理的高血压患者100例,并且分析实施1年后,血压控制情况和生活行为的改变情况。结果 所有患者中,有96例成功随访,随访率96%。相比医护卫联合规范管理实施前,管理后的收缩压显著降低[(124.5±12.1)mmHg比(151.5±15.3)mmHg],舒张压也显著低于管理前[(72.4±8.5)mmHg比(95.6±10.1)mmHg],差异均有统计学意义(均P<0.001);在遵医嘱服药、每次自测血压的比例、每日摄盐量和每周活动量上比较管理后也显著优管理于实施前,差异均有统计学意义(均P<0.001)。结论 在社区医院开展医护卫联合门诊,可以更有效地的控制高血压患者的血压水平。

Objective To evaluate the effect of Doctor-Nurse-Public Health Doctor management on blood pressure control in patients with hypertension.Methods In 2016,100 patients with hypertension were regulated with Doctor-Nurse-Public Health Doctor management in our care center,and the blood pressure control and the change of life behavior were analyzed.Results Of all the patients,96 cases were successfully followed up with the follow-up rate of 96%.The systolic blood pressure and diastolic blood pressure decreased significantly after the standardized management was implemented[(124.5±12.1)mmHg vs.(151.5±15.3)mmHg,(72.4±8.5)mmHg vs.(95.6±10.1)mmHg;P<0.001],the rate of blood pressure self testing,daily salt intake,weekly activity amount improved significantly after the standardized management was implemented(P<0.001).Conclusion In the community hospital,Doctor-Nurse-Public Health Doctor joint outpatient service can control blood pressure in patients with hypertension more effectively.
