椎弓根螺钉联合椎板关节突螺钉固定治疗腰椎骨折

Treatment of lumbar fracture with pedicle screw and laminar screw fixation

目的研究腰椎骨折患者采用椎弓根螺钉联合椎板关节突螺钉进行治疗的临床价值。方法选取2014年1月~2017年9月在本院治疗的104例腰椎骨折患者,通过随机分组的形式将其分成对照组和治疗组,每组52例。对照组采用常规开放手术方式进行治疗;治疗组采用椎弓根螺钉联合椎板关节突螺钉进行治疗。比较两组患者在手术治疗前后椎间隙高度水平的改善幅度、腰椎骨折手术治疗总有效率、术后出现不良反应的例数、术后住院治疗时间和腰椎功能恢复正常时间。结果治疗组患者在手术治疗前后椎间隙高度水平的改善幅度大于对照组,组间数据比较差异有统计学意义(P<0.05);腰椎骨折手术治疗总有效率为90.4%,对照组为65.4%,组间比较差异有统计学意义(P<0.05);术后仅出现2例不良反应,少于对照组的10例,差异有统计学意义(P<0.05);术后住院治疗时间和腰椎功能恢复正常时间短于对照组,组间比较差异有统计学意义(P<0.05)。结论腰椎骨折患者采用椎弓根螺钉联合椎板关节突螺钉进行治疗,可以在短时间内改善和恢复腰椎功能,减少不良反应,缩短住院治疗时间。

Objective To study the clinical value of pedicle screw combined with facet screw in the treatment of lumbar fractures.Methods 104 cases of lumbar fracture treated in our hospital were randomly divided into the control group and the treatment group,with an average of 52 cases in each group.The control group was treated by conventional open operation,while the treatment group was treated with pedicle screw and laminar screw.The improvement of the height of the intervertebral space in the two groups,the total effective rate of the surgical treatment of the lumbar fracture,the number of cases of adverse reaction after the operation,the time of postoperative hospitalization and the normal recovery time of the function of the lumbar spine were compared.Results The improvement of the level of intervertebral space in the treatment group was greater than that of the control group before and after the operation(P<0.05),the total effective rate of the lumbar fracture was 90.4%,the control group was 65.4%,and the difference between the groups was significant(P<0.05),and only 2 adverse reactions were found after the operation,and 10 cases were less than the control group.The difference was significant(P<0.05);the postoperative hospitalization time and the recovery time of lumbar function were shorter than those of the control group,and there was a significant difference between the two groups(P<0.05).Conclusion Pedicle screw combined with laminectomy screw can improve and restore the function of the lumbar spine in a short time,reduce the adverse reaction and shorten the time of hospitalization.
