末端可弯曲输尿管镜在输尿管上段结石治疗中的应用

Application of end flexible ureteroscope in patients with upper ureteral calculi

目的:探讨末端可弯曲输尿管镜辅助治疗输尿管上段结石的应用效果。方法:选取采用末端可弯曲输尿管镜辅助治疗的150例输尿管上段结石患者为观察组,选取同期使用输尿管镜辅助治疗的150例输尿管上段结石患者为对照组,比较两组患者的结石一次性清除率、碎石成功率、手术时间、术后住院时间、治疗费用以及并发症发生情况等指标。结果:观察组患者的结石一次性清除率和碎石成功率分别为87.3%和93.3%,分别高于对照组的68.0%(χ~2=16.162,P=0.000)和72.0%(χ~2=23.821,P=0.000);观察组输尿管上段迂曲患者的手术时间为(35.8±4.4)min、住院时间为(3.2±1.7)d、治疗费用为(8 950.3±1 202.9)元,分别低于对照组输尿管上段迂曲患者的(52.2±7.8)min(t=11.619,P=0.000)、(5.4±2.1)d(t=5.135,P=0.000)和(13 053.4±2 614.6)元(t=9.069,P=0.000)。结论:采用末端可弯曲输尿管镜辅助治疗输尿管上段结石,可以有效提高结石一次性清除率和碎石成功率,缩短患者治疗时间,降低治疗费用,值得临床推广应用。

Objective:To investigate the effect of end flexible ureteroscope(Sun's ureteroscope)in the treatment of upper ureteral calculi.Methods:150 upper ureteral calculi patients treated with Sun's ureteroscope were chosen as the observation group and 150 upper ureteral calculi patients treated with ureteroscope during the same period were chosen as the control group,with the one-time stone clearance rate,the lithotripsy success rate,the operation time,hospitalization time and treatment costs compared.Results:The one-time stone clearance rate(87.3%)and the lithotripsy success rate(93.3%)in the observation group were significantly higher than those in the control group which were 68.0%(χ2=16.162,P=0.000)and 72.0%(χ2=23.821,P=0.000)respectively.The operation time,hospitalization time and treatment costs of the observation group were(35.8±4.4)min,(3.2±1.7)d and(8 950.3±1 202.9)yuan respectively,all significantly shorter and lower than those in the control group which were(52.2±7.8)min,(5.4±2.1)(t=5.135,P=0.000)and(1 3053.4±2 614.6)yuan respectively(t=9.069,P=0.000).Conclusion:Sun's ureteroscope lithotripsy for upper ureteral calculi can effectively improve the one-time stone clearance rate and the lithotripsy success rate,shorten the operation time,hospitalization time and reduce medical costs,worthy of clinical application.
