MRI动态增强结合扩散成像对肝小血管瘤诊断价值分析

摘要 目的探究肝小血管瘤患者在使用磁共振成像(MRI)动态增强结合扩散成像后的诊断价值。方法研究对象为我院2016年11月~2017年11月收治的20例肝小血管瘤患者,根据动态增强特征将其分为典型(Ⅰ型)、高灌注型(Ⅱ型)、非典型(Ⅲ型)三类,分别观测其MRI平扫、动态增强扫描、扩散成像表现并进行定量分析,进而综合评价各型肝小血管瘤的MRI表现特点及鉴别诊断要点。结果三型肝小血管瘤在DWI中的病灶-肝实质信噪比分别为3.05、2.84、2.86,其组间数据无显著差异(F=1.16,P>0.05),而三组肝小血管瘤在ADC值上,与正常肝组织相比,均具有显著差异(t=6.89、16.61、3.46,P<0.05),其中以Ⅱ型最为显著(t=16.61,P<0.05)。结论对肝小血管瘤患者使用MRI动态增强结合扩散成像进行诊断,可通过ADC值与正常肝组织进行区分,从而能准确诊断出肝小血管瘤,此方法值得应用与推广。 Objective To analyze the clinical value of MRI(magnetic resonance imaging)dynamic enhancement and diffusion imaging in the diagnosis of hepatic hemangioma.Methods 20 patients with hepatic hemangioma treated from November 2016 to November 2017 in our hospital were selected.The subjects were divided into typical group(Type I),hypertransfusion group(Type II)and atypical group(TypeⅢ)according to the dynamic enhancement characteristics.According to the MRI plain scan,MRI dynamic enhancement scan and diffusion imaging results,the MRI manifestations and differential diagnosis value of TypeⅠ,ⅡandⅢof hepatic hemangioma were evaluated.Results The CNR(contrast-to-noise ratio)for Type I,II andⅢof hepatic hemangioma was 3.05,2.84 and 2.86,respectively(F=1.16,P>0.05);the ADC value for the lesioned tissues and normal hepatic tissues was significantly different(t=6.89,16.61,3.46,P<0.05);in particular,the ADC value for Type II of patients was most remarkable(t=16.61,P<0.05).Conclusion The MRI dynamic enhancement and diffusion imaging can improve the diagnosis accuracy of hepatic hemangioma;the ADC value for the lesioned tissues and normal hepatic tissues is significantly different.It is worthy of clinical application and promotion.
