胰腺实性假乳头状瘤的临床病理特征分析

Clinicopathologic Analysis of Solid Pseudopapillary Tumor of Pancreas

目的探讨胰腺实性假乳头状瘤的临床病理特征。方法回顾性分析3例胰腺实性假乳头状瘤患者的临床资料、病理特征以及免疫组化。结果肿瘤组织排列成实性并伴有不同程度硬化间质,肿瘤细胞围绕小血管形成假乳头状,并见出血、坏死、黏液变性及胆固醇沉积;细胞异型不明显,未见核分裂象;免疫组化检查结果:3例患者Vim、α-ATT、NSE、SYN、PR均呈阳性,S-100、AE1/AE3、EMA均呈阴性,2例患者LYS呈阳性,1例患者ER、Cg A呈阳性。结论胰腺实性假乳头状瘤作为一种胰腺低分化恶性肿瘤,应根据临床症状、病理形态学改变及免疫组化进行综合诊断。

Objective In order to explore the clinical and pathological features of solid pseudopaillary tumor of the pancreas.Methods The clinicopathologic features and immunohistochemical staining of 3 patients with solid pseudopaillary tumor of the pancreas were retrospective analyzed.Results Solid tumor tissues are arranged in with different degree of sclerotic stroma.the pseudopapillary structures were formed by tumor cells wrapping the blood vessels accompanied by haemorrhage,necrosis,mucoid degeneration and cholesterol deposition without cell heteromorphosis or mitosis.The immunohistochemical staining showed that Vim,α-ATT,NSE,SYN and PR of 3 patients were positive,S-100,AE1/AE3 and EMA were negative.LYS of 2 patients were positive,ER and CgA of 1 patient were positive.Conclusion The solid pseudopaillary tumor of the pancreas is a clinically rare poorly differentiated tumor of pancreas.We should considered fully Clinical symptoms,pathological morphology and immunohistochemical in the imaging diagnosis.
