摘要 目的探讨糖尿病前期的血脂特点及其对血糖转归的影响效果,以便为糖尿病及其并发症诊治提供可靠的依据。方法抽取本院综合内科在2014年1月~2015年12月收治的糖尿病前期患者240例作为研究对象,根据血糖状态分为IFG组(空腹血糖受损,n=50例)、IGT组(糖耐量异常,n=120例)、IFG+IGT组(空腹血糖受损+糖耐量异常,n=70例),所有患者均进行血脂水平与血糖水平检测,进行2年以上随访,调查三组患者转归情况,并进行比较分析。结果 IFG组、IGT组、IFG+IGT组三组患者在血脂水平上经统计学分析显示TC、TG、HDL-C比较差异显著(P<0.05),而LDL-C比较无显著差异(P>0.05);对三组脂代谢异常患病率进行统计学分析,显示高TG血症发生率比较差异显著(P<0.05),而高TC血症、低LDL-C血症、高HDL-C血症发生率比较无显著差异(P>0.05);三组患者随访2年时显示正常糖尿量、糖尿病比较差异显著(P<0.05),其中IFG+IGT组进展为糖尿病的百分比最高。结论糖尿病前期患者有不同程度脂代谢异常情况,高TG血症较多,其中IFG+IGT转归为糖尿病的几率最高,可见血脂对糖尿病前期患者转归有一定影响,需加强重视。 Objective To explore the characteristics of blood lipids in early stage of diabetes and its effects on blood glucose outcome,and for further providing reliable evidence for the diagnosis and treatment of diabetes and its complications.Methods A total of 240 pre-diabetes patients admitted to the comprehensive internal medicine of our hospital from January 2014 to December 2015 were enrolled in the study.According to the blood glucose status,they were divided into three groups:Impaired Fasting Glucose(IFG,n=50),Impaired Glucose Tolerance(IGT,n=120),and IFG+IGT group(n=70).All patients were tested for blood lipid and blood glucose levels and followed up for more than 2 years.Make an investigation on clinical outcome of patients in above three groups and do a comparative analysis.Results Based on blood lipid measurement,TC,TG and HDL-C were significantly different among IFG,IGT and IFG+IGT groups(P<0.05),while no significant differences were observed in LDL-C level(P>0.05).Statistical analysis of lipid metabolism abnormalities among three groups showed that the incidence of hyper triglyceridemia was significantly biased(P<0.05),while the incidence of high TC,low LDL-C or high HDL-C wasn’t significantly different(P>0.05).The follow-up of 2 years showed that patients with normal glucose tolerance and with diabetes were significantly different in three groups(P<0.05),among which the IFG+IGT group had the highest percentage of diabetes progression.Conclusion Pre-diabetes patients have different degrees of abnormal lipid metabolism,and high TGemia.Among them,IFG+IGT group has the most possibility to progress as diabetes.This indicated that blood lipid level has certain influence on the outcome of patients with pre-diabetes and should be paid more attention.
