云南地区420例皮肤恶性肿瘤回顾性分析

Retrospective analysis of 420 cases with skin malignant tumor of Yunnan

目的探讨云南地区皮肤恶性肿瘤的发病情况及临床特点。方法回顾性分析2013年到2017年昆明医科大学第二附属医院皮肤科所有行组织病理检查患者的临床资料。结果收集了420例皮肤恶性肿瘤的患者,其中单发皮肤恶性肿瘤395例,同一患者患多个皮肤恶性肿瘤25例,基底细胞癌(BCC)仍为云南最常见的皮肤恶性肿瘤;BCC、鳞状细胞癌(SCC)好发头面颈部;恶性黑色素瘤(CMM)好发于四肢末端;Bowen病发病部位可遍及全身;皮肤恶性肿瘤主要好发于老年群体,就诊高峰年龄在(61~70)岁,70岁以后有下降趋势;此次研究病例临床诊断和病理诊断符合率普遍不高,需提高对可疑癌变皮损的认识,积极行组织病理学检查。结论云南地区皮肤恶性肿瘤的发病情况较国内其他地区具有一定的自身特点,对深入进行皮肤恶性肿瘤的流行病学研究、发病机制及防治有重要的临床意义。

Objective To investigate the clinical features and trends in the incidence of skin malignant tumor in Yunnan region.Methods Clinical data of the patients diagnosed pathologically as skin malignant tumor in the department of dermatology of The Second Affiliated Hospital of Kunming Medical University from 2013 to 2017 were collected and analyzed.Results 420 cases of patients with skin malignant tumor was collected,395 cases had single skin malignant tumor,25 cases had several skin malignant tumors,basal cell carcinoma was the most common skin malignant tumor of Yunnan;Basal cell carcinoma,squamous cellcarcinoma of ten occured at head and neck;Most malignant melanomas were found at distal limbs;Bowen’s diease took place in everywhere of the body;Cutaneous malignancies were mainly found in the elderly population,the peak was 61-70 years old,and there was a decline after the age of 70;In this study,the clinical diagnosis and pathological diagnosis coincidence rate was low,so it is necessary to improve the understanding of suspected cancer lesions,and to conduct histopathological examination.Conclusion The clinical features of the patients with skin malignant tumor inYunnan region were differ from other provinces and abroad in some aspects.It has important clinical significance to make a further study of epidemiological studies,pathogenesis and prevention of skin malignant tumor.
