精神分裂症患者书写材料特点与临床特征的关系

Relationship between characteristics of writing materials and clinical features in patients with schizophrenia

目的探讨精神分裂症患者书写材料特点与临床特征的关系。方法以精神分裂症患者(32例)和双相障碍-躁狂发作患者(7例)于2016年1月至2017月4月住院前后1周书写材料作为研究对象,采用阴性和阳性症状量表(PANSS)评定精神分裂症患者的临床症状,用选定的20个维度盲法评定患者书写材料的特点。结果书写材料特点条目数与绝大部分条目出现的百分率,在伴与不伴思维散漫的精神分裂症患者中,或伴与不伴幻觉妄想的精神分裂症患者中均不具有明显的组间差异。书写材料特点仅与PANSS总分相关(r=0.36,P=0.040)。精神分裂症患者更多出现"浓缩"(62%vs 14%,P=0.035)、"怪词"(81%vs 14%,P=0.002),躁狂患者更多出现布局散乱(25%vs 85%,P=0.005)。精神分裂症患者书写材料表现出的异常方面较躁狂患者多[(8±3)vs(6±3),P=0.037]。结论评估书写材料的特点或许可以辅助诊断精神分裂症。

Objective To explore the relationship between the characteristics of writing materials and clinical features in the patients with schizophrenia.Methods The writing materials at 1 week before and after admission in 32 inpatients with schizophrenia and 7 inpatients with bipolar disorder-manic episode from January 2016 to April 2017 served as the study subjects.The Positive and Negative Syndrome Scale(PANSS)was applied to evaluate the clinical symptoms in the patients with schizophrenia.The characteristics of writing materials were assessed by 20 chosen dimensions blind method.Results The item number of writing material characteristics and the percentage of most items emerging had no obvious inter-group difference between the schizophrenia patients with and without loosening of associatron and between the schizophrenia patients with and without hallucination/delusion.The characteristics of writing materials was only correlated with the PANSS total score(r=0.36,P=0.040).The schizophrenia patients shoned more“condensing”(62%vs.14%,P=0.035)and“queer-phrase”(81%vs.14%,P=0.002),while the patients with mania shoned more“dispersed arrangement”(25%vs.85%,P=0.005).The abnormal aspects in the writing material expression in the patients with schizophrenia were more than those in the patients with mania([8±3)vs.(6±3),P=0.037].Conclusion Assessing the writing material characteristics perhaps can auxiliarily diagnose schizophrenia.
