护士对患者非计划性拔管心理体验的质性研究

Qualitative Research on the Psychological Experience of Nurses with Unplanned Extubation

目的深入了解护士对患者非计划性拔管的真实心理体验,以期为制订针对性的干预措施提供参考。方法 2018年2月,采取目的抽样法选取武汉市某三级甲等综合医院经历过患者非计划性拔管的15名护士为研究对象,采用现象学研究方法对其进行半结构式访谈,现场录音,借助NVivo 8.0软件,根据Colaizzi的7步分析法对资料进行分析,提炼主题。结果护士对患者非计划性拔管的心理体验可以归纳为2个类属8个主题,类属一:情绪体验,即害怕、内疚、惊慌无措、紧张;类属二:认知体验,即患者非计划性拔管的原因、非计划性拔管可以预防、对职业认知的变化、对处理非计划性拔管的意见。结论护理管理者须重视护士对患者非计划性拔管的心理体验,并采取针对性的干预措施。

Objective To explore the nurses’psychological experience of unplanned extubation,in order to provide a reference for establishing the targeted interventions.Methods By using purposive sampling method,15 nurses were selected and analyzed by semi-structured interview with on-site recording.Data were analyzed by 7 steps Colaizzi method with NVivo 8.0,and the themes were extracted.Results Nurses’psychological experience of unplanned extubation could be summarized into 2 categories with 8 subjects.The category one was the emotional experience,including fear,guilt,panic,nervousness;the category two was the cognitive experience,including the reason for unplanned extubation,unplanned removal can be prevented,changes in occupational cognition,suggestions on handling unplanned extubation.Conclusions Nursing managers should pay attention to the nurses’psychological experience of unplanned extubation and take specific interventions.
