新生儿及婴儿早期髋关节脱位及髋关节发育不良的X线诊断意义

X-ray diagnosis of hip dislocation and developmental dysplasia of the hip in neonates and infants

目的探讨新生儿及婴儿早期髋关节脱位及髋关节发育不良的X线诊断价值。方法将36例先天性髋关节脱位和髋关节发育不良患儿纳入观察组研究对象,将35名健康婴幼儿纳入对照组研究对象,两组入选者均接受常规X线检查,比较两组髋臼形态、髋臼指数、髋中闭孔线及股骨颈轴线检查结果,分析X线对髋关节异常的诊断效果。结果观察组髋臼形态异常率、髋臼指数异常率、髋中闭孔线阳性率、股骨颈轴线阳性率均高于对照组,差异有统计学意义(P<0.05);X线诊断髋关节异常的灵敏度为96.92%、特异度为94.29%、准确率为95.56%、阳性预测值为94.03%、阴性预测值为97.06%,诊断效果良好。结论 X线对新生儿及婴儿早期髋关节脱位及髋关节发育不良诊断准确率较高,值得临床推荐。

Objective To explore the X-ray diagnostic value of hip dislocation and developmental dysplasia of the hip in neonates and infants.Methods 36 children with congenital dislocation of the hip and dysplasia of the hip were included in the study group.35 healthy infants were included in the control group.The two groups were examined by routine X-ray examination.The results of the two groups of acetabular morphology,acetabular index,the coxa obturator and the femoral neck axis were compared,and the X-ray was analyzed.The diagnosis of abnormal hip joint.Re?sults The abnormal rate of acetabulum,the abnormal rate of acetabular index,the positive rate of the hip obturator and the positive rate of the femoral neck axis were higher in the observation group than in the control group.The difference was statistically significant(P<0.05);the sensitivity of the X-ray diagnosis of hip abnormality was 96.92%,the specificity was 94.29%,the accuracy rate was 95.56%,the positive predictive value was 94.03%,and the negative rate was 94.03%.The predictive value is 97.06%,and the diagnostic effect is good.Conclusion X-ray has high diagnostic accuracy in the diagnosis of hip dislocation and developmental dysplasia of the hip in neonates and infants,and is worthy of clinical recommendation.
