同步鼻塞式CPAP呼吸机在肺透明膜病早产儿治疗中的应用研究

Application of synchronous nasal plug CPAP ventilator in the treatment of premature infants with hyaline membrane disease

目的:探讨同步鼻塞式CPAP呼吸机(NCPAP)用于肺透明膜病(HMD)早产儿治疗中的临床效果。方法:选取HMD早产儿84例,将其随机分为观察组和对照组各42例。对照组患者实施肺表面活性物质(PS)联合有创机械通气(CMV)治疗,观察组患者实施肺表面活性物质联合NCPAP治疗,观察对比两组患儿的临床疗效。结果:两组患儿治疗前血气分析比较无统计学差异(P>0.05);治疗1h、12h后,观察组患者的血气分析均显著优于对照组(P<0.05)。观察组患者并发症发生率以及氧疗时间均显著低于对照组,而治疗成功率显著高于对照组(P<0.05)。结论:肺透明膜病早产患儿采用同步鼻塞式CPAP呼吸机治疗效果显著,可以有效改善患儿的血气分析指标情况,降低并发症发生率,缩短氧疗时间。

Objective:To explore the clinical effect of synchronous nasal plug CPAP ventilator(NCPAP)in the treatment of premature infants with pulmonary hyaline membrane disease(HMD).Methods:84 cases of HMD preterm infants were randomly divided into 42 cases in the observation group and the control group.The control group of patients with pulmonary surfactant(implementation PS)combined with mechanical ventilation(CMV)treatment,observation group patients with pulmonary surfactant combined with NCPAP treatment,to observe the clinical effect of the two groups were compared.Results:There was no significant difference in blood gas analysis between the two groups before treatment(P>0.05).After treatment of 1H and 12h,the blood gas analysis in the observation group was significantly better than that in the control group(P<0.05).The incidence of complications and the time of oxygen therapy in the observation group were significantly lower than that of the control group,and the success rate of the treatment was significantly higher than that of the control group(P<0.05).Conclusion:Pulmonary hyaline membrane disease preterm children with synchronous nasal CPAP ventilator treatment effect is remarkable,can effectively improve the blood gas analysis index of children,reduce the incidence of complications,shorten the time of oxygen therapy.
