CT引导下经皮肺穿刺肺癌微波消融术并发症的防治及护理

目的:探讨CT引导下经皮肺穿刺肺癌微波消融术(PMCT)并发症的防治及护理措施。方法:回顾性分析2015年06月-2017年12月47例在CT引导下行PMCT治疗的肺癌患者的临床病历资料,分析术后并发症的防治及护理措施。结果:47例患者顺利完成肺癌微波消融术,6个月内完全缓解(CR)25例,部分缓解(PR)12例,稳定(SD)6例,进展(PD)4例,总有效(ORR)为91.49%(43/47)。并发症8例,发生率17.02%。其中气胸3例(6.38%),发热2例(4.25%),疼痛2例(4.25%),咯血1例(2.13%)。结论:PMCT具有操作方法简单、靶向性强、局部热效率高、肿瘤局部灭活彻底、对正常组织损伤小、并发症少等优点,但对于并发症的防治和护理仍是治疗中的难点,严格掌握手术适应证,熟练操作技能,精细的围手术期护理是预防和降低并发症的发生,确保该手术疗效的重要手段。

Objective:Objective to explore the prevention and nursing measures for complications of CT guided percutaneous lung cancer microwave ablation(PMCT).Method:Methods the clinical data of 47 patients with lung cancer who were treated by CT guided PMCT were retrospectively analyzed from 06 months 2015 to December-2017.The prevention and treatment of postoperative complications and nursing measures were analyzed.Results:47 patients successfully completed lung cancer microwave ablation,complete remission(CR)in 25 cases in 6 months,partial remission(PR)in 12 cases,stability(SD)in 6 cases,progression(PD)in 4 cases,and the total effective(ORR)was 91.49%(43/47).There were 8 cases of complications,the incidence of which was 17.02%.Pneumothorax occurred in 3 cases(6.38%),fever in 2 cases(4.25%),pain in 2 cases(4.25%),and hemoptysis in 1 cases(2.13%).Conclusion:PMCT has the advantages of simple operation,strong targeting,high local thermal efficiency,complete local inactivation of tumor,small injury to normal tissue and few complications.But the prevention and nursing of complications is still the difficult point in the treatment.Strict control of surgical indications,skilled operation skills and meticulous perioperative nursing care are strict.Prevention and reduction of complications are important means to ensure the efficacy of this operation.
