潜伏梅毒患者突发性耳聋8例报告

8 Cases Report of Sudden Hearing Loss in Patients with Latent Syphilis

目的分析潜伏梅毒患者突发性耳聋的临床症状、诊疗经过及随访效果,为临床鉴别诊断与治疗提供依据。方法回顾性分析潜伏梅毒的8例(其中1例合并HIV)突发性耳聋患者的临床症状、听力结果、血清学结果、诊疗经过及预后效果。结果潜伏梅毒的突发性耳聋患者的临床特征是以单侧突发性听力下降为主,多数伴有耳鸣(6/8),少数伴有眩晕(3/8),听力损失程度和类型特征不典型;中度、平坦型听力曲线治疗后听力改善有一定效果。结论对于已知潜伏梅毒突发性耳聋患者,及早积极采取规范驱梅治疗结合突发性耳聋诊疗对部分患者听力有一定改善效果。诊疗过程中建议加强病史采集,以及梅毒血清学检测,以防漏诊误诊。

Objective To analyze the clinical symptoms,diagnosis prognosis and treatment effect in those patients with latent syphilis,suffered from sudden hearing loss and to provide the evidences for future clinical diagnosis and treatment.Methods The author retrospectively analyzed the clinical symptoms,hearing results,serological results as well as diagnosis and treatment effects from eight patients who had a sudden hearing loss with latent syphilis(including one with HIV).Results Our research showed that unilateral sudden hearing loss was the mainly features for patients who had sudden hearing loss with latent syphilis.Tinnitus was the majority presenting symptom(6/8),and a few of them had vertigo symptom(3/8).there was a lack of characteristics in the extent of hearing loss and the type of hearing loss.There was a certain improvement to the moderate hearing loss and flat hearing curve after treatment.Conclusion It was effective to perform anti-syphilis treatment combined with sudden deafness treatmenttimely for those patients with latent syphilis,suffered from sudden deafness.It is suggested to add the syphilis serological tests and to strength the collection of cases history in case of missed diagnosis or misdiagnosis.
