鼻中隔筛骨垂直板联合体在鼻综合整形术中的应用

The Use of Perpendicular Plate of Ethmoid Bone in Comprehensive Rhinoplasty

目的:探讨筛骨垂直板在鼻部综合整形术中的应用。方法:切取部分中隔软骨及筛骨垂直板,将其切为条状,分别固定于"L"形支架上,利用其做成鼻中隔延长移植物和鼻小柱支撑移植物,联合人工鼻假体材料(膨体聚四氟乙烯或硅胶)同期行鼻综合整形术。结果:本组共142例就医者,其中14例失访,7例术后笑时鼻部有支撑感,121例随访1~36个月,鼻形均得到较好改善,效果满意。结论:筛骨垂直板在鼻部综合整形术中较好地增加了鼻中隔张力,其方法简单,效果良好,值得在临床上推广应用。

Objective To investigate whether the use of perpendicular plate of ethmoid bone in comprehensive rhinoplasty can achieve a favorable aesthetic outcome.Methods An autogenous graft for extending nasal septum and columella was made by attaching resected strips of cartilage of septum and perpendicular plate of ethmoid bone to a‘L’shaped stent.Implants made of expanded polytetrafluoroethylene(ePTFE)or silicon were also used simultaneously for augmentation of the nasal tip and nasal dorsum.Results A total of 142 patients in this group,14 patients were lost to follow-up,7 patients had a postoperative complication of feeling uncomfortable when smile.121 cases achieved safe,attractive,and acceptable outcomes in nasal contouring during the 1-36 months follow-up period.Conclusion The use of perpendicular plate of ethmoid bone improved the tension of nasal septum and obtained favorable aesthetic outcomes.It is a simple and feasible therapeutic approach for comprehensive rhinoplasty in clinical practice.
