牙髓血运重建术联合MTA对年轻恒并根尖周病变患者牙根管壁厚度的影响 被引量:15

摘要 目的:研究牙髓血运重建术联合MTA对年轻恒并根尖周病变患者牙根管壁厚度的影响。方法:选取2015年10月-2017年10月笔者医院接受诊治的160例年轻恒并根尖周病变患者为研究对象,参照随机数字表法分成对照组和研究组,每组各80例。对照组接受常规的CH糊剂联合牙髓血运重建术治疗;研究组给予牙髓血运重建术联合MTA治疗。术后对两组患者随访2年,比较两组患者治疗前后患牙冠根比、牙根管壁厚度变化及牙骨质样组织沉积率、临床疗效及不良反应发生率。结果:治疗前,两组患者的牙根管壁厚度、患牙冠根比水平比较,差异无统计学意义(P>0.05);治疗后,两组患者的牙根管壁厚度、患牙冠根比水平均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);治疗后,研究组患者的牙骨质样组织沉积率高于对照组,差异有统计学意义(P<0.05);研究组患者的治疗总有效率为96.25%,高于对照组的86.25%,差异有统计学意义(P<0.05);研究组患者治疗不良反应发生率1.25%,略低于对照组的5.00%,但差异无统计学意义(P>0.05)。结论:牙髓血运重建术联合MTA治疗年轻恒并根尖周病变疗效显著,对牙根的继续发育具有促进作用,且不良反应少,值得临床推广。 Objective To study the effect of pulpotransplatation combined with MTA on the treatment of young permanent teeth complicated with periapical lesions and its effect on the thickness of root canal.Methods 160 cases of patients with periapical lesions who underwent diagnosis and treatment in our hospital from October 2015 to October 2017 were selected as the study subjects.All the patients were divided into control group and study group.According to random number table method,80 cases in each group.The patients in the control group received routine CH paste plus endodontic reconstructive surgery,and the patients in the study group received pulpotransplatation combined with MTA and were followed up for 2 years.Before and after treatment,the ratio of root to root crown,root canal wall thickness,cement deposition rate,clinical efficacy and incidence of adverse reactions in both groups were compared.Results Before the treatment,there was no significant difference in root canal wall thickness and root crown ratio between the two groups(P>0.05).After treatment,the root canal wall thickness and crown root ratio of the two groups were higher than those before treatment,and the study group was higher than the control group,the difference was statistically significant(P<0.05);After treatment,the deposition rate of cementum in the study group was higher than that in the control group,with significant difference(P<0.05).The total effective rate of the study group was 96.25%,which was significantly higher than that of the control group(86.25%,P<0.05).The incidence of adverse reactions in the study group was 1.25%slightly lower than that in the control group 5.00%,but the difference was not statistically significant(P>0.05).Conclusion Pulmonary artery reconstructive surgery combined with MTA treatment of young permanent teeth with periapical lesions significant effect on the continued development of root has a promoting effect,and less adverse reactions,is worth clinical promotion.
