摘要
目的探讨血清超敏C反应蛋白(hs-CRP)及脂蛋白相关磷脂酶A2(Lp-PLA2)水平与冠心病患者纤维脂质斑块纤维帽厚度的关系。方法选取冠心病患者97例,其中稳定型心绞痛37例、不稳定型心绞痛44例、急性心肌梗死16例。应用酶联免疫吸附法以及免疫比浊法检测患者血清hs-CRP及Lp-PLA2水平,应用光学干涉性断层成像技术测定患者的纤维脂质斑块纤维帽厚度。采用Pearson相关分析血清hs-CRP及Lp-PLA2水平与冠心病患者纤维脂质斑块纤维帽厚度的关系。结果急性心肌梗死、不稳定型心绞痛、稳定型心绞痛患者血清hsCRP、Lp-PLA2水平依次增高,纤维脂质斑块纤维帽厚度依次降低,两两比较差异均有统计学意义(P均<0.05)。纤维帽厚度<65μm患者血清hs-CRP及Lp-PLA2水平高于纤维帽厚度≥65μm患者(P<0.05)。冠心病患者血清hs-CRP及Lp-PLA2水平与冠心病患者纤维脂质斑块纤维帽厚度均呈负相关(r分别为-0.624、-0.598,P均<0.05)。结论冠心病患者血清hs-CRP、Lp-PLA2水平与纤维脂质斑块纤维帽厚度相关,此两项指标检测可有效评估患者冠状动脉粥样硬化斑块状态。
Objective To study the relationship of serum hypersensitive c-reactive protein(hs-CRP)and lipoprotein associated phospholipase A2(Lp-PLA2)levels with the thickness of fibrous cap of fibrolipid plaques in patients with coronary heart disease.Methods Totally 97 patients with coronary heart disease were selected,including 37 cases of stable angina pectoris,44 cases of unstable angina pectoris,and 16 cases of acute myocardial infarction.ELISA and immunoturbidimetry were used to detect the serum hs-CRP and Lp-PLA2 levels,the thickness of fibrous cap fibrolipid plaques in patients was measured by Optical interferometric tomography.The correlation of serum hs-CRP and Lp-PLA2 levels with the thickness of fibrous cap of fibrolipid plaques in patients with coronary heart disease was analyzed by Pearson correlation analysis.Results The levels of serum hs-CRP and Lp-PLA2 in patients with acute myocardial infarction,in patients with unstable angina pectoris and in patients with stable angina pectoris increased gradually,with statistically significant difference(all P<0.05).The thickness of fibrous cap of fibrolipid plaques in patients with stable angina pectoris,in patients with unstable angina pectoris,and in patients with acute myocardial infarction increased gradually,with statistically significant difference(all P<0.05).The serum hs-CRP and Lp-PLA2 levels in patients with the thickness of fibrous cap<65μm was higher than those with the thickness of fibrous cap≥65μm(P<0.05).The level of serum hs-CRP and Lp-PLA2 in patients with coronary heart disease was negatively correlated with the thickness of fibrous cap of fibrolipid plaques(r=-0.624,-0.598,both P<0.05).Conclusion The serum levels of hs-CRP and Lp-PLA2 in patients with coronary heart disease are related to the thickness of fibrous cap of fibrolipid plaques,and the detection of two clinical indicators can effectively assess coronary atherosclerotic plaques in patients.
