摘要
目的探讨剪切波速度的变异系数诊断乳腺癌的价值。方法对109个乳腺肿块行剪切波弹性成像,分别测量肿块内最高、最低、中等剪切波速度,根据均数和标准差计算变异系数,并比较乳腺良恶性肿块变异系数的差异。结果乳腺恶性肿块剪切波速度的变异系数明显高于乳腺良性肿块(0.302±0.093vs 0.167±0.057;t=9.361,P<0.001)。以剪切波速度的变异系数=0.225为界值诊断乳腺恶性肿块的敏感度为85.00%(34/40),特异度为89.86%(62/69),阳性预测值为82.93%(34/41),阴性预测值为91.18%(62/68)。结论乳腺剪切波速度变异系数能反映乳腺肿块的离散度,是鉴别乳腺良恶性肿块的一个重要指标。
Objective To explore the value of shear wave elastography(SWE)dispersion in diagnosis of malignant breast lesions.Methods A total of 109 breast lesions were examined with SWE.The high,low,mediate shear wave velocity(SWV)values of the lesions were obtained,and the coefficient of variation was measured basing on the mean and standard deviation.Differences of SWV coefficient of variation wer compared between benign and malignant breast lesions.Results The coefficient of variation of malignant breast lesions was higher than that of benign lesions(0.302±0.093 vs 0.167±0.057;t=9.361,P<0.001).Taking SWV coefficient of variation=0.225 as the cut-off value,the sensitivity,specificity,negative predictive value and positive predictive value in diagnosis of malignant breast lesions was 85.00%(34/40),9.86%(62/69),91.18%(62/68)and 82.93%(34/41),respectively.Conclusion SWE is accurate to reflect the dispersion of breast lesions,therefore can be used as an important indicator for differential diagnosis of benign and malignant breast lesions.
