射血分数保留心力衰竭患者血清半乳糖凝集素-3水平变化及意义 被引量:5

Changes and significance of serum galectin-3 level in patients with heart failure with preserved ejection fraction

摘要 目的探讨半乳糖凝集素-3(galectin-3)在射血分数保留心力衰竭(HFp EF)诊断中的临床意义,并与N-末端脑钠肽前体(NT-proBNP)的诊断价值进行比较。方法选取四川省南充市第二人民医院于2016年1月~2017年1月住院并确诊为HFp EF患者35例(实验组),同期选没有左心室收缩及舒张功能障碍的心衰患者35例(对照组),通过ELISA试剂盒测定galectin-3与N-末端脑钠肽前体(NT-proBNP)水平,并比较galectin-3和NT-proBNP在HFp EF患者中的诊断价值。结果 Galectin-3和NTproBNP在HFp EF患者中均增高(P<0.001),在HFp EF的患者中的galectin-3与NT-proBNP、左心室体积指数(LVMI)、左心房容积指数(LAVI)、早期二尖瓣流速/舒张早期延长速度(E/E’)呈正相关性(P<0.001);galectin-3的ROC曲线下面积为0.98,NT-proBNP的ROC曲线下面积为1。结论半乳糖凝集素-3是射血分数保留心衰患者诊断中的一个重要的生物标记物,值得在临床大力开展。 Objective To discuss the clinical significance of serum galectin-3 level in diagnosis of heart failure with preserved ejection fraction(HFpEF),and compare the diagnostic value between galectin-3 and N-terminal pro-brain natriuretic peptide(NT-proBNP).Methods HFpEF patients(n=35,test group)and patients with heart failure and without systolic and diastolic dysfunction(n=35,control group)were chosen from the Second People’s Hospital of Nanchong City of Sichuan Province from Jan.2016 to Jan.2017.The level of NT-proBNP was detected by using ELISA,and diagnostic value was compared between galectin-3 and NT-proBNP in diagnosis for HFpEF patients.Results The levels of galectin-3 and NT-proBNP all increased in HFpEF patients(P<0.001).The levels of galectin-3 and NT-proBNP were positively correlated to left ventricular mass index(LVMI),left atrial volume index(LAVI)and early mitral flow velocity/early diastolic mitral flow velocity(E/E’)in HFpEF patients.The area under ROC of galectin-3 was 0.98 and that of NT-proBNP was 1.Conclusion Galectin-3 is an important biomarker in diagnosis for HFpEF patients,which is worth clinical promoting.
