脂蛋白相关磷脂酶A2、血清胱抑素C、脂蛋白(a)与冠脉狭窄的相关性

Correlation Analysis of Lp-PLA2,Cys C,Lp(a) and Coronary Artery Stenosis

目的探讨冠心病患者脂蛋白相关磷脂酶A2(lipoprotein associated phospholipase A2,Lp-PLA2)、血清胱抑素C(Cystatin C,,Cys C)水平、脂蛋白(a)[Lipoprotein(a),Lp(a)]与冠心病患者冠脉狭窄程度的关系,进一步预测冠脉病变程度。方法回顾性分析我院151例行冠脉造影患者,明确为冠心病患者112例作为冠心病组,39例冠脉造影示冠状动脉狭窄<50%的患者为对照组。根据Gensini评分,冠心病组分为轻、中、重度病变三组,根据冠脉病变支数,分为单支、双支、多支病变组,分别于术前测定Lp-PLA2、Lp(a)、Cys C水平,分析Lp-PLA2、Lp(a)、Cys C与冠脉狭窄程度相关性。结果 Gensini评分重度组Lp-PLA2、Cys C、Lp(a)水平高于中度组、轻度组及对照组。多支冠脉病变组高于双支、单支病变组,差异有统计学意义(P<0.05)。Lp-PLA2、Cys C、Lp(a)水平与冠脉Gensini评分、冠状动脉病变支数呈正相关(P<0.05)。结论血清Lp-PLA2、Cys C、Lp(a)水平越高,冠脉狭窄程度越严重,与冠脉狭窄程度呈正相关,监测血清Lp-PLA2、Cys C、Lp(a)水平对预测冠脉狭窄程度有一定的临床意义。

Objective To study the relationship between lipoprotein phospholipase A2,lipoprotein a,cystatin C and severity of coronary artery lesions in patients with coronary artery disease.Methods 151 patients undergone coronary angiography in our hospital were retrospectively analyzed.112 patients were diagnosed coronary heart disease(CHD)as coronary heart disease group.39 patients were selected as control group by coronary angiography whose coronary artery stenosis were lower than 50%.According to the Gensini score,the patients were divided into 3 grops:low score group,moderate score group and high score group.The patients were divided into 3 groups according to the severity of coronary artery stenosis:single-vessel group,double-vessel group and multiple-vessel group.Lp-PLA2,Lp(a),Cys C were checked before the Operation.Correlation analysis of Lp-PLA2,Lp(a),Cys C and severity ofcoronary disease was analyzed.Results The concentration of Lp-PLA2,Cys C,Lp(a)in high score group was significantly higher than that Moderate score group,Low score group and control group.The concentration of Lp-PLA2,Cys C,Lp(a)in multiple-vessel group was significantly higher than that singlevessel group,double-vessel group(P<0.05).Lp-PLA2,Cys C,Lp(a)were positively associated with Gensini score and lesion count of coronary artery(P<0.05).Conclusion The higher the level of Lp-PLA2,Cys C,Lp(a),the severity of coronary artery lesions is higher,and they have a positive correlation.Detection of serum concentrations of Lp-PLA2,Cys C and Lp(a)can evaluate severity of coronary artery lesions.
