氩氦刀冷冻消融与手术切除治疗原发性肝癌的临床疗效

The Clinical Efficacy of Argon-helium Knife Cryoablation and Surgical Resection for Primary Liver Cancer

目的探讨氩氦刀冷冻消融与手术切除治疗原发性肝癌的临床疗效,为寻找肝癌最佳治疗方案提供参考依据。方法收集2012年4月至2014年4月接受治疗的原发性肝癌患者62例,其中采用手术切除术治疗的患者30例(手术组)和氩氦刀冷冻消融治疗的患者32例(氩氦刀组),比较手术时间、术中失血量、术后住院时间及围手术期并发症(发热、疼痛、腹腔出血及感染),以及术后生存率、生存时间及复发率。结果氩氦刀组患者手术时间、术中失血量及术后住院时间均优于对照组,差异有统计学意义(P<0.05)。氩氦刀组患者术后疼痛、腹腔出血及感染发生率低于手术组,差异有统计学意义(P<0.05);二者术后发热率、术后生存率、平均生存时间及复发率相似,差异无统计学意义(P>0.05)。结论氩氦刀冷冻消融与手术切除治疗原发性肝癌的术后生存率及复发率相似,但是氩氦刀冷冻消融治疗手术创伤小、并发症少、住院时间短、生存质量高。

Objective To explore the clinical efficacy of argon-helium knife cryoablation and surgical resection for primary liver cancer,providing references for the best treatment of liver cancer.Methods From April 2012 to April 2014,62 cases of primary liver cancer patients were selected.Among them,30 patients were treated with surgical resection(surgical group)and 32 patients were treated with argon-helium knife cryoablation(argon-helium knife group).Compare the surgical time,intraoperative blood loss,postoperative hospitalization and perioperative complications(fever,pain,abdominal hemorrhage and infection),and the postoperative survival rate,survival time and recurrence rate.Results The surgical time,intraoperative blood loss and postoperative hospitalization of the argon-helium knife group were all significantly superior to that in the control group,and the difference was statistically significant(P<0.05);the postoperative pain,abdominal hemorrhage and infection rate of the argon-helium knife group were all significantly lower than that of the control group,and the difference was statistically significant(P<0.05);While the postoperative fever rate,postoperative survival rate,survival time and recurrence rate between the two groups were all similar,and there was no significant difference between the two groups(P>0.05).Conclusion The postoperative survival rate and recurrence rate between argon-helium knife cryoablation and surgical resection are similar,but the argon helium knife cryoablation can make less traumatic,less complication,shorten hospital stay and increase the quality of life.
