高频超声及肌电诱发电位联合在臂丛神经病变中的诊断价值

摘要 目的探讨高频超声及肌电诱发电位联合在臂丛神经病变中的诊断价值。方法研究对象方便选取该院2016年1月—2017年1月收治的疑似臂丛神经病变患者54例,所有患者均行高频超声及肌电诱发电位检查。比较两种方法单独或联合诊断臂丛神经病变结果,同时比较两种方法单独或联合诊断臂丛神经病变的特异性、灵敏度及准确率。结果高频超声、肌电诱发电位联合诊断臂丛神经病变的阳性率(88.89%)明显高于高频超声(57.70%)(χ~2=16.333,P=0.000)和机电诱发电位(55.56%)(χ~2=14.954,P=0.000)单独诊断,差异有统计学意义(P<0.05);高频超声、肌电诱发电位联合诊断臂丛神经病变的特异性(χ~2=9.818,P=0.001;χ~2=12.247,P=0.000)、灵敏度(χ~2=10.581,P=0.001;χ~2=9.375,P=0.002)和准确率(χ~2=7.337,P=0.007;χ~2=9.600,P=0.002)明显高于两种方法单独诊断。结论高频超声及肌电诱发电位联合诊断臂丛神经病变比单一检测方法诊断更有优势,可有效提高诊断灵敏度和准确率,值得临床推广使用。 Objective To investigate the diagnostic value of high frequency ultrasound and myoelectric evoked potential in the diagnosis of brachial plexus.Methods A total of 54 patients with suspected brachial plexus neuropathy convenient treated in the hospital from January 2016 to January 2017 were selected.All patients underwent high-frequency ultrasound and myoelectric evoked potential examination.Comparison of the two methods alone or in combination to diagnose brachial plexus neuropathy results,and to compare the specificity,sensitivity,and accuracy of brachial plexus neuropathy diagnosed by either method alone or in combination.Results The rate of high-frequency ultrasound and myoelectric evoked potential combined diagnosis of brachial plexus neuropathy(88.89%)was significantly higher than that of high-frequency ultrasound(57.70%)(χ^2=16.333,P=0.000)and electromechanical evoked potential(55.56%)(χ^2=14.954,P=0.000),the difference was statistically significant(P<0.05),and the specificity of high-frequency ultrasound and myoelectric evoked potentials in the diagnosis of brachial plexus neuropathy(χ^2=9.818,P=0.001;χ^2=12.247,P=0.000),sensitivity(χ^2=10.581,P=0.001;χ^2=9.375,P=0.002)and accuracy(χ^2=7.337,P=0.007;χ^2=9.600,P=0.002)was significantly higher than the two methods alone.Conclusion The combined use of high-frequency ultrasound and myoelectric evoked potentials for the diagnosis of brachial plexus neuropathy is more advantageous than a single detection method.It can effectively improve the sensitivity and accuracy of diagnosis and is worthy of clinical application.
