复杂胫骨平台骨折给予单、双侧钢板内固定术治疗的比较分析

Comparative Analysis of the Treatment of Complex Tibial Plateau Fractures with Single and Double Plate Internal Fixation

目的对比分析复杂胫骨平台骨折给予单、双侧钢板内固定术治疗效果。方法以2009年1月—2017年12月为病例选取时间,方便选取62例析复杂胫骨平台骨折患者,按照患者入院登记先后顺序分为参照组、研究组,各31例,分别予以予单、双侧钢板内固定术治疗,比较手术指标、膝关节功能恢复情况、并发症发生情况,综合对比不同固定术的应用效果。结果研究组患者手术时间明显长于参照组,而负重时间、住院时间、骨折愈合时间明显短于参照组,对比差异有统计学意义(t=8.202、8.306、12.644,P<0.05)。研究组患者膝关节功能恢复优良率为93.55%(29/31),明显高于参照组的67.74%(21/31),对比差异有统计学意义(χ~2=6.613,P=0.010<0.05)。两组患者术后并发症发生率对比为9.68%(3/31):6.45%(2/31),差异无统计学意义(χ~2=0.218,P=0.641>0.05)。结论双侧钢板内固定术治疗复杂胫骨平台骨折,虽然手术时间长,但术后康复快,能够提升膝关节功能恢复效果,且不增加并发症发生风险,效果理想,在临床中的推广应用价值高。

Objective To compare and analyze the effects of single and double plate internal fixation for complex tibial plateau fractures.Methods From January 2009 to December 2017 as the time of case selection,62 patients with complex fractures of tibial plateau were included conveniently.According to the order of admission of patients,they were divided into a reference group and a study group.Each of the 31 cases was given a single and a double lateral plate internal fixation was performed to compare the surgical indexes,the recovery of knee joint function and the occurrence of complications,and to comprehensively compare the application effects of different fixations.Results The operative time of the study group was significantly longer than that of the reference group,and the weight-bearing time,length of stay,and fracture healing time were significantly shorter than those of the reference group,with significant differences(t=8.202,8.306,12.644,P<0.05).The excellent and good rate of knee joint function recovery in the study group was 93.55%(29/31),which was significantly higher than that of the reference group[67.74%(21/31)],and the difference was significant(χ^2=6.613,P=0.010<0.05).The postoperative complication rate of the two groups was 9.68%(3/31)vs 6.45%(2/31),and there was no significant difference(χ^2=0.218,P=0.641>0.05).Conclusion Bilateral plate internal fixation for complex tibial plateau fractures,although long operation time,but the postoperative recovery is faster,can improve the effect of knee joint function recovery,and does not increase the risk of complications,the effect is satisfactory,with high clinical value of popularization and application.
