64层螺旋CT低剂量与常规剂量对早期肺癌诊断的对比

Comparison of Low-dose and Conventional Doses of 64-slice Spiral CT in the Diagnosis of Early-stage Lung Cancer

目的对比分析低剂量、常规剂量64层螺旋CT在早期肺癌诊断中的临床应用价值。方法方便选取该院2014年3月—2017年12月收治的120例早期肺癌患者,所有患者均通过病理诊断证实,所有患者均分别采用常规剂量64层螺旋CT诊断(常规剂量组)和低剂量64层螺旋CT诊断(低剂量组),对比两组早期肺癌检出率、各种征象检出率以及辐射剂量等情况。结果低剂量组早期肺癌检出率(90.0%)和常规剂量组(91.7%)差异无统计学意义(χ~2=1.674,P>0.05)。常规剂量组、低剂量组对病灶的空洞征、胸膜凹陷征、毛刺征、分叶征等征象扫描结果差异无统计学意义(P>0.05)。常规剂量组有效m As、CTDlw、最大有效辐射剂量、DLP等指标均高于低剂量组,差异有统计学意义(P<0.05)。结论低剂量、常规剂量64层螺旋CT扫描在早期肺癌诊断中均具有较高的应用价值,不同剂量对早期肺癌病灶征象扫描结果相当,但低剂量CT扫描有利于减少辐射剂量,更安全,值得推广。

Objective To compare the clinical value of low-dose and conventional-dose 64-slice spiral CT in the diagnosis of early lung cancer.Methods A total of 120 patients with early-stage lung cancer who were admitted to the hospital from March 2014 to December 2017 were convenient selected.All patients were confirmed by pathological diagnosis.All patients were diagnosed with conventional dose 64-slice spiral CT(conventional dose group)and low-dose 64 spiral CT diagnosis(low-dose group)was performed to compare the detection rate of early stage lung cancer,the detection rate of various signs,and radiation dose.Results There was no significant difference in the detection rate of early stage lung cancer(90.0%)and conventional dose group(91.7%)in low-dose group(χ2=1.674,P>0.05).There was no statistically significant difference in the scanning results of the signs of voids,pleural plunging,burr,and lobulation in the conventional and low dose groups(P>0.05).The effective doses of mAs,CTDlw,maximal effective radiation dose,and DLP were higher in the conventional dose group than in the low dose group(P<0.05).Conclusion Low-dose and conventional-dose 64-slice spiral CT scan has a high application value in the diagnosis of early lung cancer.Different doses have similar results for early lung cancer lesion scans,but low-dose CT scan is beneficial to reduce radiation dose,and it is safer,which is worth promotion.
