沙丁胺醇联合布地奈德雾化吸入对老年支气管哮喘急性发作的应用效果

Application effect of Salbutamol combined with Budesonide aerosol inhalation in the treatment of acute attack of elderly bronchial asthma

目的评价沙丁胺醇联合布地奈德雾化吸入治疗老年支气管哮喘急性发作的临床效果。方法 70例支气管哮喘急性发作老年患者,根据治疗方法不同分为对照组和观察组,各35例。对照组进行支气管哮喘常规治疗,给予氨茶碱静脉滴注和沙丁胺醇雾化吸入;观察组在对照组基础上给予布地奈德雾化吸入。观察比较两组患者治疗前后血气指标[动脉血二氧化碳分压(PaCO_2)、动脉血氧分压(PaO_2)]及肺功能指标[第1秒用力呼气容积(FEV1)、第一秒用力呼气容积占用力肺活量的比值(FEV1/FVC)],并比较两组患者的治疗效果。结果治疗前,两组患者PaCO_2、PaO_2、FEV1、FEV1/FVC水平比较差异无统计学意义(P>0.05);治疗后,观察组患者的PaCO_2水平低于对照组,PaO_2、FEV1、FEV1/FVC水平高于对照组,差异具有统计学意义(P<0.05)。观察组治疗总有效率为97.1%,对照组治疗总有效率为80.0%,观察组治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。结论老年支气管哮喘急性发作给予沙丁胺醇联合布地奈德雾化吸入治疗,可有效改善患者肺功能及血气指标,缓解临床症状,提高治疗效果。

Objective To evaluate the clinical efficacy of Salbutamol combined with Budesonide aerosol inhalation in the treatment of acute attack of elderly bronchial asthma.Methods A total of 70 patients with acute attack of elderly bronchial asthma were divided into control group and observation group according to different treatment methods,35 cases in each group.The control group was given bronchial asthma conventional therapy,including intravenous infusion of Aminophylline and aerosol inhalation of Salbutamol,the observation group was given Budesonide aerosol inhalation on the basis of the control group.The blood gas indexes[arterial partial pressure of carbon dioxide(PaCO2),arterial partial pressure of oxygen(PaO2)],pulmonary function indexes[forced expiratory volume in 1st second(FEV1),ratio of forced expiratory volume in 1st second to forced vital capacity(FEV1/FVC)]before and after treatment between the two groups were observed and compared,and the therapeutic effect was also compared.Results Before treatment,there were no significant differences in the levels of PaCO2,PaO2,FEV1 and FEV1/FVC between the two groups(P>0.05).After treatment,the level of PaCO2 in the observation group was significantly lower than that in the control group,and the levels of PaO2,FEV1 and FEV1/FVC in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of the observation group was 97.1%,that of the control group was 80.0%.The total effective rate of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion Salbutamol combined with Budesonide aerosol inhalation in the treatment of acute attack of elderly bronchial asthma can effectively improve pulmonary function indexes and blood gas indexes,alleviate clinical symptoms and improve therapeutic effect.
