远隔缺血处理对慢性脑灌注不足的神经保护作用研究 被引量:2

摘要 越来越多的研究关注缺血处理(ischemic conditioning)对急性脑缺血/再灌注损伤的神经保护作用。根据处理的时间不同,缺血处理分为缺血前处理(ischemic pre-conditioning)、缺血过程中处理(ischemic per-conditining)和缺血后处理(ischemic post-conditioning),分别在脑缺血事件发生前、发生过程中或发生后对脑组织进行一系列短暂的阻断血流/再灌注,以激活多种内源性保护机制,减轻脑组织损伤。由于其临床应用受到对脑实施额外缺血风险的限制,近些年来,缺血处理的概念扩大至远隔缺血处理(remote ischemic conditioning,RIC),通过袖带加压阻断肢体血流之后再复流,可以反复多个循环,也能发挥对脑组织的保护作用。由于RIC的无创性,而且肌肉组织对缺血的耐受性较好,已经由临床试验证实对急性脑缺血是有效的[1]。理论上讲,RIC对慢性脑灌注不足的患者同样应该有效,并已经被研究证实[2,3],然而相关的研究数量较少,而且关于RIC在慢性脑灌注不足过程中发挥保护作用的机制尚不完全清楚。
