前程常规分割联合后程大分割适形放疗鼻咽癌的临床分析

Clinical analysis of the conventional segmentation combined with post-procedural segmentation conformal radiotherapy for nasopharyngeal cancer

目的研究大分割三维适形放疗鼻咽癌的放射损伤及疗效。方法采用倾向性得分匹配法,把105例鼻咽癌患者分成全程常规分割适形放疗组(对照组)和前程常规分割后程大分割三维适形放疗组(研究组),观察放射损伤、近期疗效、远期疗效等。结果两组急性放射损伤、远期放射损伤差异均无统计学意义(P> 0.05),治疗后3个月完全缓解率研究组(88.5%)与对照组(67.9%)对比差异有显著性(χ2=6.471,P <0.05),5年生存率研究组(78.8%)、对照组(60.4%)亦有明显优势(χ2=4.225,P <0.05)。结论前程常规分割联合后程大分割适形放疗用于鼻咽癌患者的近期疗效、远期疗效较好,且放射损伤反应与全程常规分割适形放疗并无明显差异,安全性较好。

Objective To investigate the radiation damage and curative effect of nasopharyngeal carcino-ma with large segmented three-dimensional conformal radiotherapy.Methods According to the propensity score matching method,one hundred and five cases of NPC patients in our department were divided into the routine conventional segmentation conformal radiotherapy group(the control group)and the future normal segmentation posterior segmentation three-dimension conformal radiotherapy group(the study group).Radiation damage,short-term efficacy and long-term efficacy were recorded.Results There were no significant differences in the acute radiation injury and the long-term radiation injury between the two groups.The complete remission rate post 3-month treatment(88.5%)was significantly different from that of the control group(67.9%)(χ2=6.471,P<0.05),the 5-year survival rate in the study group(78.8%)and in the control group(60.4%)also had significant advantages(χ2=4.225,P<0.05).Conclusion The short-term curative effect and the long-term curative effect of the conventional segmentation combined with posterior segmentation conformal radiotherapy for nasopharyngeal carcino-ma are good,with no obvious difference between the radiation injury response and the conventional segmentation conformal radiotherapy,as well as with good safety.
