益气活血平肝汤联合赖诺普利对高血压肾病患者血液流变学、尿蛋白、肾功能和血管内皮功能的影响

Effect of Yiqi Huoxue Pinggan Decoction combined with lisinopril on hemorheology,urine protein,renal function and vascular endothelial function in patients with hypertensive nephropathy

目的观察益气活血平肝汤联合赖诺普利对高血压肾病患者血液流变学、尿蛋白、肾功能和血管内皮功能的影响。方法将148例高血压肾病患者随机分为2组,对照组74例给予赖诺普利治疗,研究组74例给予益气活血平肝汤联合赖诺普利治疗,2组均持续治疗2个月。比较2组治疗后临床疗效,检测2组治疗前后血液流变学、尿蛋白、肾功能和血管内皮功能指标水平。结果治疗后,研究组临床治疗总有效率显著高于对照组(P <0. 05); 2组高切全血黏度、纤维蛋白原、血细胞比容、尿蛋白水平及血清肌酐(SCr)、血尿素氮(BUN)、血管紧张素Ⅱ(AngⅡ)和内皮素-1(ET-1)水平均较治疗前显著降低(P均<0. 05),且研究组均显著低于对照组(P均<0. 05); 2组血清一氧化氮(NO)水平均较治疗前显著升高(P均<0. 05),且研究组显著高于对照组(P <0. 05)。结论益气活血平肝汤联合赖诺普利治疗高血压肾病能明显改善血液流变学指标,增强肾功能和血管内皮功能。

Objective It is to observe the effects of Yiqi Huoxue Pinggan Decoction combined with lisinopril on hemorheology,urinary protein,renal function and vascular endothelial function in patients with hypertensive nephropathy.Methods A total of 148 patients with hypertensive nephropathy were randomly divided into two groups.74 patients in the control group were treated with lisinopril.74 patients in the study group were treated with Yiqi Huoxue Pinggan Decoction combined with lisinopril.Both groups were treated for 2 month.The clinical efficacy of the two groups was compared,and the levels of hemorheology,urinary protein,renal function and vascular endothelial function were measured before and after treatment.Results After treatment,the total effective rate of clinical treatment in the study group was significantly higher than that in the control group(P<0.05).The levels of high blood viscosity,fibrinogen,hematocrit,urinary protein and serum creatinine(SCr),blood urea nitrogen(BUN),angiotensinⅡ(AngⅡ)and endothelin-1(ET-1)were significantly lower than those before treatment in the two groups(P<0.05),and the levels of these indicators in the study group were significantly lower than those in the control group(P<0.05).The levels of serum nitric oxide(NO)in the two groups were significantly higher than those before treatment(P<0.05),and the level in the study group was significantly higher than that in the control group(P<0.05).Conclusion Yiqi Huoxue Pinggan Decoction combined with lisinopril can significantly improve hemorheology and enhance renal function and vascular endothelial function in patients with hypertensive nephropathy.
