摘要
目的探讨驱动蛋白超家族中kif14在结肠癌中的表达。方法选取81例结肠癌组织标本(结肠癌组)及30例癌旁组织标本(正常对照组)为研究对象,免疫组化染色检测kif14蛋白在结肠癌及癌旁组织中的表达并对其临床相关性进行分析。结果结肠癌组阳性表达45例(55. 6%),正常对照组阳性表达6例(20. 0%),两组阳性率比较,差异有统计学意义(P <0. 05)。kif14与结肠癌肿瘤直径、淋巴结转移具有相关性(P <0. 05),而与其他特征无显著相关性(P> 0. 05)。本组81例结肠癌样本中,kif14高表达患者总体存活率显著低于kif14低表达患者,差异有统计学意义(P <0. 05)。在单因素Cox风险回归模型中,肿瘤的分化程度、肠壁浸润深度、淋巴结转移数、TNM分期、kif14与生存期具有相关性。多因素Cox回归分析显示,肿瘤分化程度、TNM分期、kif14可作为结肠癌的预后因子预测患者的生存期。结论 kif14在结肠癌组织中的表达明显高于正常组织,其可能在结肠癌的发生发展中起重要作用。kif14基因的高表达通常与患者的不良预后具有相关性,其可能是潜在的预测结肠癌预后的独立因子。
Objective To investigate the expression of kif14 and clinical significance in colorectal cancer.Methods A retrospective study was performed on 81 cases of specimens of colorectal cancer tissues(colorectal cancer group)and 30 cases of specimens of adjacent tissues(normal control group).Immunohistochemical staining was used to detect the expression of kif14 in colorectal cancer and adjacent tissues and to analyze its clinical correlation.Results Positive expression of colorectal cancer group was in 45 cases(55.6%),positive expression of normal control group was in 6 cases(20.0%),positive rate comparison between the two groups had statistically significant difference(P<0.05).The kif14 was correlated with tumor diameter and lymphnode metastasis of colorectal cancer(P<0.05),while had no significant correlation with other features(P>0.05).Among all the 81 cases of colorectal cancer specimens,the overall survival rate of patients with over-expression of kif14 was significantly lower than that of the patients with lower-expression of kif14,and the differences were statistically significant(P<0.05).In the Univariate Cox regression model,the degree of tumor differentiation,the depth of intestinal wall infiltration,the number of lymph node metastases,TNM stage and kif14 were correlated with survival.Multivariate Cox regression analysis showed that tumor differentiation,TNM stage and kif14 could be used as prognostic factors of colorectal cancer to predict survival.Conclusion The expression of kif14 in colorectal cancer is significantly higher than adjacent tissues,which may play a critical role in colorectal cancer carcinogenesis.Over-expression of kif14 has a significant correlation with poor prognosis of colorectal cancer,which may be a potential independent prognosis predictor of colorectal cancer.
